Cooperative for Human Services Inc. is a multi-service organization that provides residential services, independent living supports, and guardianship services for adults with intellectual, developmental and other related disabilities. We are seeking a compassionate leader to join our team as a full time Clinical Coordinator! This role involves providing clinical services to the people we serve consistent with the organization's philosophy, approach, and other interrelated clinical practices.

Schedule: Monday - Friday 9 AM - 5 PM

Job Description

  • In collaboration with the CHS clinical team, develop & implement clinical interventions, strategies, approaches, & plan designed to enhance the lives of the people we serve
  • Attend & actively participate in clinical meetings, clinical appointments, ISP meetings, family meetings, etc. as necessary
  • Promote & protect the rights and dignity of each person served & extend these rights & dignities to family members & guardians
  • Continually monitor support relationships for potential abusive or neglectful situations
  • Develop data collection forms/charts; collect, review, analyze, data to ensure the efficacy of current clinical interventions and plans
  • Provide monitoring and follow-up with program managers to ensure interventions and strategies are implemented in accordance with the clinical plan and are effectively addressing support needs
  • Communicate in a positive, professional manner with the individuals served, family members/guardians, other service providers, management personnel, psychologist/psychiatrists, funding & referral sources, & stakeholders of the organization
  • Ensure compliance with local, state, and federal laws and regulations as well as the policies and procedures of CHS, DDS regulatory requirements, & QUEST diagnostics
Requirements:
  • Bachelors degree in a related field
  • 5 years of relevant clinical experience & experience working directly supporting people with developmental disabilities or another another protected population
  • Excellent verbal & written communication skills
  • Demonstrated ability to work as part of a team
  • valid driver's license
  • CPR/FA certification
