Job Summary:
An individual that performs non-specialized tasks such as bathing, dressing, hygiene, toileting and feeding. Completes required documentation related to nursing policy and procedures. The tasks completed will be under the supervision of a licensed nurse.

Major Responsibilities, Duties, Functions, and Tasks:

• Assists residents with personal care needs based on their wishes and routine as well as their care planned interventions
• Makes beds
• Positions residents for comfort, using cushions and supportive devices, as needed
• Provides preventative skin care (based on care planned information in the resident care guide) in hopes of preventing skin breakdown
• Assists residents with ambulation, transfers and frequent changes in position
• Prepares residents for meals and provides care after the meal
• Uses mechanical lifts per the resident transfer/lift forms and per facility policy and procedure
• Uses gait belts for any ambulatory resident requiring contact guard assistance or greater unless care planned otherwise
• Checks on residents frequently, especially those who are unable to call for help or communicate their needs
• Interacts with Team Members, Residents, and Guests in a courteous and professional manner
• Full Time Part Time Status is required to work every other weekend:
