POSITION SUMMARY:

For those who have come to reside permanently, we aim to create the best possible environment while optimizing their quality of life. EPIC Rehabilitation and Nursing at White Plains is a **NEW** Skilled Nursing & Rehabilitation center located in the heart of White Plains. This SNF is part of our " Epic Healthcare Management" group of facilities.
- The primary purpose of the job position is to provide assigned residents with routine daily nursing care in accordance with established nursing care procedures, and as may be directed by supervisors.
ESSENTIAL JOB RESPONSIBILITIES: During the course of your shift you may be required to perform some or all of the following:
- Administrative Functions:
- Accurately record all entries in the electronic medical records system throughout the shift
- Use the wristband to identify residents before serving meals and delivering care.
- Report all accidents and incidents you observe on the shift on which they occur, to the charge nurse or supervisor.
- Report all changes in resident’s condition to the charge nurse as soon as it occurs.
2. Admission/discharge functions:
- Ensure that the resident’s room is ready for receiving the resident. (i.e. bed made, name tags up, admission kit available etc.).
- Greet and escort residents to their room. Introduce residents to his/her roommate, if any, and other residents and personnel as appropriate. Make resident comfortable.
- Inventory and mark the resident’s personal possessions. Assist residents with packing their personal possessions when they are being transferred or discharged.
- Transport residents to new rooms or to receiving area. Assist with loading/unloading as necessary.
3. Personnel Functions:
- Follow work assignments, and/or work schedules in completing and performing assigned task. Perform all tasks in accordance with established policies and procedures, and as instructed by supervisors.
- Notify the facility when you will be late or absent from work, according to facility policy.
- Cooperate with inter-departmental personnel, as well as other facility personnel to assure that nursing services can be adequately maintained to meet the needs of the residents.
- Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the unit and shift.
4. Personal nursing care functions:
- Participate in and receive the nursing report upon reporting for duty.
- Assist residents with daily dental and mouth care.
- Give or assist residents with bath functions.
- Assist residents with dressing/undressing as necessary.
- Assist residents with nail care. Shave male residents.
- Assist residents with hair care.
- Keep residents dry.
- Make beds. Put extra covers on beds as requested.
- Position residents in correct and comfortable position.
- Assist residents with bowel/bladder functions. Maintain intake and output records as instructed.
- Assist residents in preparing for activity and social programs.
- Assist with transportation.
- Assist with lifting, turning, moving, positioning, and transferring residents into and out of beds, chairs, bathtubs, wheelchair, lifts, etc.
- Answer resident calls promptly.
- Perform restorative and rehabilitative procedures as instructed.
- Provide eyes care and ears care.
- Measure and record temperatures, pulse, respirations, and blood pressures as instructed.
- Weigh and measure residents as instructed. Reports weight loss/gain to supervisor.
- Assist with the care of the dying resident.
- Check residents routinely to assure that their personal care needs are being met.
- Performs other duties as requested.
5. Special nursing functions:
- Observe resident’s skin closely during care and report the presence of pressure/red areas or areas of breakdowns.
- Willing to work beyond normal working hours, on weekends and holidays, and in other positions temporarily, when necessary.
- Provide daily indwelling catheter care.
- Provide daily perineal care.
- Provide daily range of motion exercises.
- Turn bedfast residents at least every two hours.
- Provide residents with reality orientation as instructed.
6. Food service functions:
- Prepare residents for meals such as take to bathroom, wash hands, raise bed, position tables, place bibs, and take to dining room.
- Serve food trays. Assist with feeding as indicated and needed.
- Monitor diet order and diet served for errors.
- Assist residents with sight problems to identify food arrangements on tray.
- Record the resident’s food/fluid intake. Report changes in the resident are eating habits. Assist resident to receive alternative tray if resident does not like the food served or does not eat it.
- Keep resident water pitchers clean and filled with fresh water (on each shift) and within easy reach of the resident.
- Perform after meal care such as removing trays, cleaning resident’s hands, face, and clothing.
- Monitor diet orders and restrictions, and make sure resident does not receive foods they are restricted from having due to medical reasons or due to allergic reactions.
7. Staff development:
- Attend and participate in scheduled training and educational classes. (Must attend at least 6 hours per 6 months). Training will be a combination of live in-services and computer in-services.
- Attend quarterly fire drills on your assigned shift.
- Make suggestions to supervisors on topics that may help educate people in delivering quality care.
8. Safety and Sanitation:
- Check on residents frequently - at least every 30 minutes.
- If a resident is care planned for a restraint, release restraint at least every two hours for range of motion exercises, take to bathroom.
- Notify charge nurse if resident is missing.
- Keep floors dry, report or wipe up spills immediately.
- Keep the call system within easy reach of the resident.
- Do not leave work equipment and supplies in work area, store in appropriate areas when not in use.
- Wash hands before and after performing any service for the resident.
- Follow established Contact Precautions and Procedures. Always follow Universal Precautions.
- Report any communicable or infectious disease that you contract to the Director of Nursing Services.
- Perform routine housekeeping duties (i.e. clean over bed table, keep drawers and closets neat and clean, etc.)
9. Care Plan Functions:
- Review care cards daily to ensure that you are providing appropriate nursing care.
- Inform Charge Nurse of any changes that need to be made on the care plan.
- Ensure that your notes reflect that the care plan is being followed.
- Participate in planned care conferences.
10. Residents Rights Functions:
- Maintain the confidentiality of all resident care information.
- Ensure that you treat all residents fairly, and with kindness, dignity, and respect.
- Ensure that all nursing care is provided in privacy. Knock before entering the resident’s room, wait for approval from the resident before entering.
- Report all grievances and complaints made by the resident to the Charge Nurse.
Education:
- Minimum High School Diploma (or equivalent).
- Certification as a Nurse Aide in the State of New York
- Must be current and in good standing with NYS CNA Certification.
Work Experience:
- No working experience required beyond clinical experience required to become certified
- Previous long term care or skilled nursing experience a plus!
Job Knowledge/Requirements:
- Excellent communication and organization skills
- Speak English in an understandable manner. Knowledge of other languages may be helpful.
- Ability to work well under pressure and be flexible.
- Able to relate to and work with the ill, disabled, elderly, emotionally upset, and sometimes challenging individuals within the facility.
- Have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents
- Have the ability and willingness to work harmoniously with professional and non-professional personnel.
Working Conditions/Physical Requirements (ADA):
- Good general health and demonstrate emotional stability.
- Able to lift a minimum of 50 pounds.
- Able to cope with the mental and emotional stress of the position.
- Works in well-lighted and ventilated areas as well as throughout the nursing service area.
- Sits, stand, bends, lifts, and moves intermittently during working hours.
- Subject to hostile and emotionally upset residents, family members, personnel, and visitors.
- Subject to odors and exposure to infectious waste, diseases, and conditions throughout the workday.
