Job description
SpiriTrust Lutheran is now hiring a Center Manager - LIFE
*8:00am-4:00pm Monday through Friday*
Required:
- Must be a graduate of an applicable bachelor’s or master’s degree program
- Registered Nurse or Social Worker degree preferred, or possesses a suitable combination of education and experience
Must attain and maintain CPR certification
Experience: Five years of applicable experience with at least two years of supervisory experience and one year of experience working with a frail or elderly population required.
Eligibility for Employment: Must meet eligibility requirements to work in a Medicare/Medicaid environment including criminal background and other eligibility clearances, clearance for communicable diseases and all required immunization must be current.
General Responsibilities:
- Provides leadership and facilitates the interdisciplinary care team meetings for development and implementation of care plans
- Ensures that documentation of team meetings is maintained and changes are recorded in individual care plans
- Ensures that notification is sent to appropriate agencies and regulatory bodies when required
- Ensures documentation in medical records is complete and current.
