Responsibilities:

The CBHI (Children’s Behavioral Health Initiative) is the umbrella program for ICC (Intensive Care Coordination) and HBT (Home Based Treatment). The CBHI Intake Coordinator provides support and coordination of services and advocacy to parents/caregivers enrolled in the program. The Intake Coordinator ensures the youth’s and family’s voice is heard throughout the wraparound process and facilitates the provision of creative, customized services and supports to youth and families that enhance their lives and lead to positive outcomes.

  • Serve as the single point of entry for CBHI referrals
  • Triage referrals appropriately, following the ICC/HBT Timely Access to Care Protocol.
  • Maintain a referral log and waitlist for CBHI services and meet twice weekly with Program Directors to review and triage as needed.
  • Make required bi-weekly phone calls to families on the waitlist.
  • Communicate with appropriate Behavioral Health Plan after a youth who has been on the CBHI waitlist for 14 days.
  • Attend and actively participate in weekly individual and group supervision to review referrals, manage waitlist (as needed), review progress of youth/families served, identify risk factors, and improve communication/ interagency collaboration, and foster professional growth.
  • Work with other BAMSI programs and the local MCI, to provide coordinated and comprehensive crisis diversion and intervention services.
  • Maintain records, reports, and documentation for families served, including authorizations, Individual Care Plans, Treatment Plans, Crisis Plans, Weekly Progress Notes, Assessments, and required Evaluation tools.
  • Conduct clinical practice in accordance with BAMSI policies, all state department regulations, Masshealth regulations, the requirements of the Board of Registration in Medicine under the Patient Care Assessment Program, and any other regulatory agencies to which these programs are subject.
  • File 51As (Child Abuse and Neglect Report) with support from supervisor as required by mandated reporting laws.
  • Maintain regular and timely attendance, professional boundaries, quality clinical service and high professional standards at all times.
  • Adhere to the agency’s policy regarding confidentiality, and execute duties to reflect reasonable safety standards.
  • Universal/standard precautions must be utilized and training obtained in areas that constitute risk.
  • Perform other related work duties as needed or as assigned by supervisor or designee

Qualifications:

EDUCATION/CREDENTIALS:

  • Master's degree (in required field, per MCE list of approved degrees) and a minimum of five (5) years of experience working with the target population pursuant to MCE credentialing criteria; licensure preferred


EXPERIENCE:

  • Experience in navigating any of the child/family-serving systems and experience advocating for family members who are involved with behavioral health systems


KNOWLEDGE/SKILLS/ABILITIES:


  • Working knowledge of the Wraparound Process and Systems of Care Principles
  • Must demonstrate sensitivity to cultural diversity and an ability to accept families, children and youth unconditionally without judgment or blame for the challenges they face.
  • Ability to communicate effectively with service providers and administrators, to work independently and as part of a team, and to embrace the System of Care Principles and Family Systems Theory.
  • Must possess excellent care coordination, group facilitation, computer, time management and organizational skills.
  • Must be flexible and able to respond to families under stress and in crisis.
  • Must have valid driver’s license in state of residence and own means of transportation
