SUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITY
- Provide remote community-based services and support to Individuals regarding self-advocacy, education, community awareness, medication management and support.
- Provide web-based primary and behavioral health integrated services
- Respond to the needs, desires, and interests of Individuals utilizing formal and informal assessment practices
- Provide weekly web-based interventions to individuals.
CLIENT'S REQUIRED EXPERIENCE & EDUCATION
- Minimum of a Bachelor's Degree in Social Work
- Michigan Social Worker License in good standing (LLBSW/LBSW LLMSW/LMSW)
- QMHP Certification (preferred)
