Minimum Requirements:

  • RN, LPN or Bachelors' Degree in related field + one year of paid professional experience with the aging or disable population
  • ADvantage Case Management certificate preferred

Pay and Benefits:

  • Salary $22 - $26 per hour (competitive pay offered for a certified ADvantage Case Manager)

Description:

The Advantage Case Manager performs the core functions of comprehensive assessment, service planning, service plan implementation, monitoring and reassessment for members receiving Advantage services. These services are performed under the directions and supervision of Advantage Case Manager Supervisor.

Qualifications:

  • Personal vehicle needed for travel and a valid driver's license.
  • Works independently but performs as a member of a team
  • Knowledge of Federal and State regulations as well as agency policy and procedures
  • Effective communication skills, both oral and written.
  • Ability to deal with a variety of personalities and situations.
  • Integrity to maintain confidentiality of information.
  • Exhibits active listening skills

Responsibilities:

  • Conducts the admission of members
  • Obtains a medical history, particularly as it relates to the present condition of the member and or family member
  • Evaluates the member, family member(s) and home situation to determine what services will be needed
  • Evaluates the member's environment to determine what assistance will be available from family members and other informal supports in caring for the member
  • Evaluates the member's condition and home situation to determine if other services, such as those of a home health aide, will be required and the frequency of this service
  • Interprets nursing and other services to members and the families as a part of planning for care.
  • Conducts activities to develop the member's service plan and coordinate services authorized.
  • Submits case management progress notes and other outlining the services rendered as indicated.
  • Submits a tally of visits/units billed each day.
  • Cooperates with other agencies providing using or related services to provide continuity of care and to implement member's service plan
  • Develops service plans and service plan addendum that facilitate cost effective home and community based services for optimal outcomes for members
  • Perform all other duties as assigned
