Community Healthlink Community Healthlink (CHL) is looking for a full-time (40 hrs/wk) Case Manager in Worcester, MA.

Adult Outpatient Services at Community Healthlink
provides integrated primary and behavioral health care services in our outpatient clinics for our consumers with severe and persistent mental illness and/or a chronic substance use condition. The integrated model includes nurse care managers who coordinate care with the behavioral and primary care providers, perform wellness assessments, develop wellness treatment plans and develop and facilitate wellness activities. Clinic staff include primary care clinicians, licensed psychologists, licensed independent clinical social workers, licensed mental health counselors, drug and alcohol counselors, psychiatrists, advanced practice psychiatric nurses, nurses, medical assistants, and administrative support staff. The clinic offers open access to services daily for our consumers and our priority population. The clinic offers substance addiction treatment including medication assisted treatment using evidence-supported models.
Responsibilities:
  • Serve as residential counselor for specified number of residents as requested by supervisor.
  • Develop, implement, and document treatment needs assessments, treatment plan, and progress reviews as part of continuous quality improvement activities.
  • Work with residents to assist with activities of daily living and foster a sense of safety in the community which includes personal hygiene and self-care skills, money management and budgeting, symptom management, relapse prevention, use of public transportation and transporting, problem solving and conflict resolution, and other interventions, as necessary.
  • Ensure healthful menu planning, food shopping, and meal preparation through coordination and assistance.
  • Monitor residents’ medication, refills, documentation of medications and changes, assist with attending appointments, and safe storage.
  • Lead two or more skill development or recreational group (s) per week as assigned.
  • Attend teams and case conferences and collaborate with other providers for designated residents.
  • Provide outreach as designated to community-based residents.
  • Prepare daily log entries, incident reports and other required paperwork to ensure the program meets the standards.
  • Work with residents to maintain the residence on a daily basis including chores, bedroom upkeep, yard, and other light housekeeping duties.
  • Perform other related job duties as assigned in order to provide quality rehabilitation services.
  • Human Rights, dual diagnosis, infection control, CPR, First Aid, MAP, agency and divisional policy and procedures as well as other required competencies.
  • May include transporting residents in personal car or program vehicle, physical assistance, and close supervision.
  • Must be able pass required trainings including Medication Administration Program (MAP), CPR and First Aid.

Qualifications:
  • Grade C02 Case Manager I: Associates degree in Human Services or HSD or HISET.
  • Grade C04: Case Manager II: Requires a Bachelor’s degree; preferably in the field of psychology or human services.
  • Current valid US-issued driver’s license and ability to provide a registered, inspected, and insured automobile for work related purposes including transporting clients in a personal vehicle
