We are looking for a dedicated Case Manager to join our mental health and addiction team!
Our agency provides behavioral health and substance abuse recovery services including but not limited to psychiatric services including medication assisted treatment for behavioral health and substance abuse treatment needs; behavioral health nursing services; individual therapy; group treatment; intensive outpatient treatment; mental health day treatment; and case management services.
Scheduled work hours: This is an hourly position. Normal agency business hours are 9:00 am - 5:30 pm, M-F.
Competencies: All AGENCY positions are expected to demonstrate competency in the areas of 1) Confidentiality; 2) Cultural Sensitivity; 3) Universal Precautions and 4) Client Rights. Additional competency standards may be required for each position based on the annual plan.
Credentials (Education/Experience/Licenses):
Education: High school diploma or GED. College preferred.
Experience: While experience is preferred, we will train someone the right candidate. We are looking for someone who is organized, has a sense of professional pride with an eagerness to learn, a sincere desire to help others, and seeks an an opportunity for career development.
Must have a valid Ohio driver’s license. Must be COVID-19, fully vaccinated.
ESSENTIAL FUNCTIONS/JOB DESCRIPTION
- Performing a case management functions such as linking, referring, coordinating, and managing the delivery of services to the client in relation to the established goals and objectives in their treatment plan.
- Developing an Individualized Service Plan (ISP) for case management and medsom services.
- Facilitating client’s utilization of community resources as appropriate by accompanying and/or assisting with the transport of clients to those resources as necessary.
- Provide direct services at a rate that achieves or exceeds the Practice’s baseline productivity indicator.
- Provide and engage in other services necessary to support treatment to clients such as clinical case meetings, staff development, maintaining clinical and administrative records per agency requirements.
- Assist clients with understanding, providing, and using health insurance benefits, including the use of publicly funded services.
- Promote a positive image of the Practice through his/her dress, demeanor, and interactions with clients, other employees and the community.
- Maintains documentation per Practice quality assurance procedures and established performance standards.
- Promotes safety and quality care throughout department and agency.
- Conducts other duties as assigned.
