McAngel Home Visiting Nurses is looking for a Case Manager who will also complete Start of Care visits. The Registered Nurse must have some home health experience (within the last 10 years) including complete proficiency with filling out the OASIS form. The Registered Nurse would be doing Start of Cares and case management. The pay and benefits are generous for this Registered Nurse position.

RN with Home Care experience ONLY

This job would entail:

Responsibilities:

• Assessment of patient and conditions as per homecare guidelines
• Collaborate with physicians and nurses to devise individualized care plans for patients
• Perform Start of Care visits as need for patients
• Comfortable with using EMR
• Case management
