Compassionate and dependable caregivers are needed to provide various personal care and related services in accordance with an established plan of care. We serve individuals and families and pride ourselves that our Home Care Partners are compassionate, competent and qualified caregivers who provide excellent quality care to our client population.

We are currently hiring for Home Care Companions and Personal Care Attendants to join our "Home Care Partner" program. Home Care Partners provide services to individuals in their own homes and communities who need assistance caring for themselves as a result of old age, sickness, disability and/or other clinical reasons. Home care may include light housecleaning, laundry, meal preparation, transportation, companionship, respite and advice on such things as nutrition, cleanliness and household activities. Personal Care may include assistance with the activities of daily living, physician appointments, meal preparation, transportation, companionship and respite. Our Home Care Partners collaborates directly with our Nurse Care Managers supporting our team effort to improve quality of life for the individuals we serve.

Both Home Care Companions and Personal Care Attendants are responsible for ensuring that service is delivered in a caring and respectful manner, in accordance with relevant Company policies and industry standards.

Both Home Care Companions and Personal Care Attendants report to the Nurse Care Manager and Client Care Coordinator.

Job Requirements
We are looking for individuals on a full-time, part-time and "as required call-out basis" to provide various in-home services including personal care, companionship/sitter, caregiver respite, friendly reassurance and intermittent chore/home maintenance.

Applicants must meet the following criteria:

• Certified Nursing Assistant or Home Health Aid Preferred
• Must be people oriented
• Have excellent interpersonal skills
• Hold a valid Driver's License
• Have reliable transportation
• Consent to undergoing a criminal background check
• Have a cell phone
• Be well groomed
