Day - 08 Hour (United States of America)
Provides appropriate care to patients and their families in a compassionate and professional manner, within the Care Coordinator scope of practice as outlined in University HealthCare Alliance (UHA) policies and procedures. Assists the patients care team in implementing the medical plan of care.
Distinguished Characteristics:
Care Coordinator - Works under the direction of licensed medical professionals and performs a variety of clerical and patient care duties to assist practitioners and other members of health care team. For situations where the Care Coordinator is designated as a "trainee," the incumbent will work under close supervision, requiring mentor sign-off on certain activities.

REQUIRED:
1. High School graduate or GED
2. Completion of an accredited Medical Assistant Training Program
3. Current CPR certification
4. 1-year related experience in ambulatory care environment

PREFERRED:
1. Certification as a Medical Assistant:
a. AAMA - American Association of Medical Assistants
b. CCBMA - California Certifying Board of Medical Assistants
c. AMT - American Medical Technologists Certification as a Registered Medical Assistant
(RMA)

What you will do
  • Assists the patients’ care team in implementing the plan of care for the patient in a collaborative and professional manner.
  • Patient Care:
  • Perform care management activities:
  • Initiating Post Discharge Calls as identified by CRN/RN CM.
  • Setting up/coordinating Follow up appointment with PCP & Specialist post hospital/SNF stay.
  • Assist RN CM/NP in data gathering/organization.
  • Provide non-medical support to Clinical Staff and Members.
  • Facilitate patient access to appropriate medical and specialty providers as well as other care coordination team support specialists.
  • Collect data and write brief reports to meet evaluation needs of the program.
  • Perform regular visits to provide patient and family support and education.
  • Under the supervision and direction of the clinical team, complete route telephonic patient case calls.
  • When necessary, accompany and support the clinical team with in-home and in-clinic patient visits.
  • Communication and Documentation:
  • Accurately collects and transmits information between the patient and provider in a courteous, respectful and professional manner, within Care Coordinator scope of practice; refers care outside of the Care Coordinator scope of practice to the appropriate licensed personnel.
  • Communicates updates to patient requests.
  • Documents appropriate information clearly, accurately and in a timely manner, according to department and UHA policies and procedures.
  • Administrative Duties:
  • Performs administrative duties in a timely, accurate and collaborative manner according to policy and procedures within the Care Coordinator scope of practice, including, but not limited to:
  • Retrieves and processes voicemails and incoming faxes refills.
  • Prepares forms and manages prior authorizations throughout the shift.
  • Epic environment: Assists providers with their in-baskets and open charts; monitor other MD and MA in- baskets when assigned.
  • All other duties as assigned including department-specific functions and responsibilities:
  • Performs other duties as assigned and participates in organization projects as assigned.
  • Adheres to safety, HIPAA and compliance policies.

Education Qualifications
  • High School graduate or GED.

Experience Qualifications
  • Completion of an accredited Medical Assistant Training Program.
  • One (1) year related experience in ambulatory care environment.

Required Knowledge, Skills and Abilities
  • Verbal and written communication skills demonstrate courtesy, compassion and helpfulness in a professional manner towards employees, patients and patient families.
  • Basic high school math, English aptitudes and legible handwriting.
  • Basic Windows skills to include keyboarding, mouse movement and computer data entry skills to enter patient information.
  • Organizational and multi-tasking skills.
  • Knowledge of medical terminology/anatomy and EMR systems.
  • Ability to work with others in a flexible, cooperative and collaborative manner.
  • Requires concentration to handle varying procedures and interruptions.

Base Pay Scale: Generally starting at $26.16 - $33.35 per hour
The salary of the finalist selected for this role will be set based on a variety of factors, including but not limited to, internal equity, experience, education, specialty and training. This pay scale is not a promise of a particular wage.
