Job description
Position Summary:
- The Behavioral Health Navigator (BHN) is part of the Hospital’s Behavioral Health Emergency team that provides a screening and assists with disposition plans for adult and pediatric patients that present in the ED with a mental health crisis
- This position helps behavioral health patients connect with resources that allow them to better manage their mental illness, improve their overall health and avoid preventable hospital encounters
- The BHN/Case Manager embedded in the ED is responsible for assisting with post discharge care and connecting patients to community resources and supports for individuals with complex need
Shift:
Full Time (40 hours) - 4 days, 10 hrs per day
Saturday - Tuesday
8:00am - 6:30pm
Weekend Work Required
Education/Experience:
1. Bachelor’s Degree with an interest in behavioral, community or public health.
2. Experience working in a hospital or clinic setting preferred
3. Experience working as part of a multi-disciplinary team as well as with people of diverse backgrounds preferred.
