Tillamook County's Health and Human Services Department is seeking a Behavioral Health Clinician to join their team. The goal is to fill this position with the most competitive candidate as a Behavioral Health Clinician 2 or 3 based on the chosen candidate's level of experience.

Meets criteria as a Qualified Mental Health Professional according to OAR291-124-1030: (2)(g) Whose education and experience demonstrate the competency to identify precipitating events; gather histories of mental and physical disabilities, alcohol and drug use, past mental health services and criminal justice contacts; assess family, social and work relationships; conduct a mental status examination; document a multiaxial DSM diagnosis; write and supervise an individual plan of care; conduct a mental health assessment and provide individual, family or group therapy within the scope of their training.
Care is provided to individuals and their families in a patient-centered primary care home or behavioral health home by licensed primary care clinicians, behavioral health clinicians and other care team members working together to address one or more of the following: mental illness, substance use disorders, health behaviors that contribute to chronic illness, life stressors and crises, developmental risk/conditions, stress-relate physical symptoms, preventative care, and ineffective patterns of health care utilization.
Behavioral Health Clinician 2
(Duties assigned to this position are not all inclusive. Additional duties may be assigned.)
In collaboration within an integrated model of care and as a member of the senior leadership team, develops, implements and evaluates an integrated and evidence-based behavioral health treatment program; identifies needs and recommends new and revised program services; develops and recommends policies, procedures, goals and priorities; coordinates services with other clinic and health department programs.- Improve population health outcomes by ensuring patients of all ages have access to behavioral health expertise as a usual part of primary care.
- Maintains a visible presence in the clinic and attends clinic meetings.
- Provides consultation to primary care providers both onsite and by phone.
- Is available for same day and scheduled initial consultations with referred patients.
- Performs brief, limited follow-up visits for selected patients.
- Provides a range of services including screening for common conditions, assessments and interventions related to chronic disease management.
- Conducts risk assessments as indicated.
- Provides psychoeducation for patients during individual or group visits.
- Assists in the development of clinical pathways, group medical appointments, classes and behavior focused practice protocols.
- Provides brief behavioral and cognitive behavioral interventions for patients.
- Triages patients with severe or high-risk behavioral health problems and refers such patients to community resources or specialty Behavioral Health (BH) services.
- Provides same-day feedback to the primary care provider and/or care team.
- Facilitates and oversees referrals to specialty BH services, and when appropriate, supports a smooth transition from specialty BH services to primary care.
- Documents consultative notes for the medical record outlining assessment findings, interventions delivered, and recommendations made to the primary care provider.
- Maintains clinical records and other necessary paperwork in a timely manner to comply with all administrative regulations.
- Educates the primary care team in the basic principles of brief behavioral and cognitive behavioral interventions and reinforces their use in the medical visit.
- Provides assistance in capturing program evaluation and fidelity measures.
- Complete all required documentation regarding client services, progress, and concerns, including crisis screening forms, as needed, Electronic Health Records, etc., in a professional and timely manner.

- Collaborate closely with the primary care providers to address integrated health care needs with the patients of the CHC, and coordinate care between primary care, integrated behavioral health, and other behavioral health services.
- Provide individual, family, and/or group services to individuals with mental health and/or substance abuse conditions in accordance with the OARs. This may include, but is not limited to case management services, outreach and referral to community resources, and helping clients develop and utilize natural supports.
- Complete all required documentation regarding client services, progress, and concerns, including crisis screening forms, as needed, Electronic Health Records, etc., in a professional and timely manner.

- Comprehensive understanding of the interrelationship between medical and psychological systems and their impact on health.
- Skill and expertise in conducting group and individual psycho-educational classes and brief, short-term therapy strategies that will help strengthen the patient’s overall physical and mental health.
- Incorporation of self-management, home-based practice in care planning.
- Knowledge of consultative interventions that help replace maladaptive behaviors with adaptive ones.
- Experience in documenting clinical activities in a comprehensive, clear, concise fashion on a same day basis.
