Job description

Full or part-time Nurse Practitioner or Physician Assistant needed for 2 outpatient Adult Internal Medicine / Primary Care roles with a Community Health group in New Haven, Connecticut. PA and NP providers with 1-2 years of Adult Internal Medicine, Adult Primary Care, or Urgent Care clinical experience will be considered. Medical Spanish a plus. This position is available as of 12/9/2022.

Nurse Practitioner / Physician Assistant will:

• Provide Adult Internal Medicine services at 1 outpatient clinic setting in New Haven, Connecticut.
• Coordinate with other clinical and non-clinical staff to provide optimal patient outcomes.
• Manage patients' acute and chronic conditions.
• Prescribe medication for patients as required or directed.
• Coordinate with specialists.
• Provide education to patients and family members when necessary.

Days / Hours / Compensation / Benefits:

• 40 weekly hours - very flexible schedule
• Nine (9) 4 hour shifts / One (1) 4 hour administrative shift
• Very competitive salary
