Job Summary:
The Admission and Discharge Planner supports the administrative needs relevant to the clinical care of incoming, current, and discharging patients and families. The Admission and Discharge Specialist works in collaboration with the Intake, Clinical, Medical, Admin and Dietary teams at THIRA Health.


Essential Job Duties:

  • Communicate with prospective patients to confirm receipt of Intake Paperwork and remind of appointment time; Respond to patient inquiries and assist patients with completion of paperwork; support Intake Team by expectation-setting with patients and families on what to expect during assessment, when to look for meeting link, and other communication as needed
  • Support with scheduling intake assessments and completing initial phone screens as part of admissions process
  • Liaise with Intake, Program Coordinator, and PAS team to ensure patients are appropriately prepared for Orientation day
  • Manage admission documents and ensure availability off all necessary records in patient chart for use by interdisciplinary team
  • Meet with patients and families during Orientation to discuss aftercare planning process and confirm need for outpatient providers or other referrals
  • Support of THIRA patient-facing team with set up of discharge and aftercare planning documentation
  • Regularly maintain referral list to ensure accuracy of provider contact information, specialties, insurance and availability; partner with Marketing team with outreach efforts to establish and maintain positive community relationships
  • Track referrals out to maintain equitable dispersion of referrals to THIRA’s preferred outpatient providers
  • Liaise with patient-facing team to support with referral information as needed and initiate set up of aftercare plan at patients’ 2/3 point in treatment
  • Coordinate with Program Coordinator on confirmed discharges and provide discharge paperwork to patients on their last day of treatment; follow up with patients electronically as needed; ensure outpatient providers receive records following a patient’s discharge from THIRA
  • Support THIRA Health’s clinical and medical quality improvement efforts as it relates to coordination of care and referrals
  • Identify patient barriers to care such as transportation, language, insurance, etc. and coordinate with patient-facing team to refer patients as needed to additional resources to support engagement in care
  • Track navigation services, record messages from patients, and contribute to clinic tracking workflows
  • Track appropriate patient-level quality outcomes for program monitoring and evaluation


Requirements:


  • Associate in relevant field (psychology, social work, healthcare administration, etc.) required bachelor’s degree in relevant field preferred Requires 1-2 years of experience with health care clinic operations; previous experience in patient-facing role highly preferred
  • Ability to work collaboratively with teams, staff and external providers and organizations
  • Experience PM and EMR applications highly preferred; experience or familiarity with medical practice software, EMR, and MicroMD preferred
  • Strong verbal and written English communication skills
  • Maintain confidentiality
  • Strong computer literacy and working knowledge of Microsoft Office applications
  • CPR & First Aid certified and/or willing to undergo CPR & First Aid certification process
  • Must be able to successfully pass a criminal background check
