About the Job
- Process all referrals in a timely manner and maintain close working relationship with referral sources
- Develop expertise in developing home and community services, contracting for services as determined for the Plan of Care
- Interact regularly with the Director of IFPS and Case Workers to ensure appropriate providers are identified to meet the specific needs of the family, the providers are following principles of culture-based wraparound and IFPS model, and the providers are meeting the clinical and operational standards set by their contract
- Attend weekly team meetings
- Develop, maintain and expand provider network; attend Montgomery County providers’ network meetings or other pertinent meetings to familiarize new providers with IFPS services
- Assist Director with database and reporting
- Bachelor’s degree with concentration in Social Sciences or Human Services
- 1-3 years’ experience
- Skilled at collaborating with multiple agencies
