We are seeking a Treatment Coordinator to coordinate Home-Based services for children who have special needs. As a Treatment Coordinator, you will be responsible for:
- Protecting the rights and dignities of individuals with developmental disabilities and extending these rights and dignities to family members or guardians
- Coordinating Home-Based Children’s Services in accordance with EOHHS guidelines
- Engaging in all aspects of treatment coordination on behalf of a pre-determined caseload of clients and assisting in whatever level necessary to meet the outlined goals in the service plan
- Meeting with all relevant caregivers, supervisees, and others involved in the plan of care to gather information and collect data for the purposes of Treatment Coordination and Services
- Managing data and preparing documents to ensure timely completion of progress reports and annual renewal reports for all assigned clients
- Providing direct treatment coverage as needed to support the ABA Center-Based program
- Providing supervision and direction to ABA, HBTS, PASS, and Respite direct support staff as well as participating in the evaluation process by assessing and reviewing overall job performance
- Assisting in the training for new and existing ABA, HBTS, PASS, and Respite direct support staff including, but not limited to data collection methods, timesheet completion/ submission, and policies/procedures. Ensuring all assigned employees complete all necessary trainings within the required time frames
- Providing feedback to ABA, HBTS, PASS, and Respite direct support staff regarding time sheets, data collection systems, attendance, punctuality, and other administrative responsibilities directed by the Director of Home-Based Children’s Service program
- Communicating with all team members to ensure that services and supports are appropriate and meet the client’s needs. Ensuring all team members are updated on family successes/concerns and schedule changes. Notifying families of any staffing changes.
- Conducting interviews with potential candidates and completing all documentation required by HR
- Coordinating with insurance providers and/or The Executive Office of Health and Human Services (EOHHS) for the maintenance of treatment services, including all necessary paperwork
- Conducting weekly insurance eligibility checks to assure reimbursement of services rendered
- Entering all coordination of billing on a weekly basis, entering all timesheets into the billing system for the direct support staff, and following up with direct support staff on any missing data/time sheets
- Performing other duties as directed by the supervisor or department director that are related to the organization’s mission, goals, and operations
Education/ Requirements:
- Bachelor’s Degree in Human Services or related major with commensurate experience is required
- Minimum of two years Human Services experience including hands-on activity in the individual planning process
- Excellent communication and writing skills
- Bilingual preferred
- Knowledge of client centered planning processes and statewide service options
- Ability to work cooperatively and reliably with coworkers, consumers, and all other individuals involved in providing supports
- Must be able to generate positive, open, and effective relationships with coworkers, consumers, families, EOHHS, insurance providers, and other professionals.
- Must demonstrate flexibility, creativity, and follow through in responding to the needs of individuals and their families
- Must successfully pass BCI and CANTs screenings
- Must have valid driver’s license, safe driving record, automobile insurance, and access to vehicle to be used as directed and when needed for transport
