Sentara Health Plans is hiring a Temporary Care Coordinator for the Harrisonburg Virginia area!
Area includes: Harrisonburg, Bridgewater, Staunton, Waynesboro
This temporary position is set to last a minimum of 6 months but has the possibility of extending or becoming permanent!
This position requires both in person face-to-face assessments and remote telephonic assessments!
As a Care Coordinator you will perform a variety of casework duties and provide case management services to patients, families, and designated caregivers. You will help develop, participate and monitor multidisciplinary collaboration of services to patients where appropriate. Assist adult patients and their families with personal and environmental difficulties associated with medical conditions up to and including at time of terminal illnesses.
Minimum Qualifications:
- 4-year degree in Health and Human Services
- 1 year of Long Term Care experience required
- Case Management/ Care Coordination experience strongly preferred
- Experience with Community Health Resources strongly preferred
- Experience initiating care plans and both subjectively and objectively conducting assessments
- Experience with educating members with chronic disease(es) strongly preferred
