POSITION TITLE: Support Services Counselor
POSITION SUMMARY:
Full-time, non-exempt, hourly; Monday through Friday; 40 hours per week. Under the supervision of the Director of Programs, the Support Services Counselor shall provide support for caregiver services, information and referrals, and assessment completion.
PRIMARY DUTIES:
Responsibilities are oriented around three main objectives:
1. Caregiver Services
a. Meet with caregivers who are burdened with the responsibility of caring for loved ones with dementia or physical disabilities.
b. Complete risk matrix to determine whether caregiver is eligible for respite services.
c. Complete assessments for caregivers to receive respite and/or supplemental services.
d. Generate authorization letters to caregivers and providers for respite grants and communicate effectively with LOS and Director of Programs to ensure proper documentation and tracking of Respite dollars/hours/Service Units
e. Identify any opportunities for caregivers to utilize supplemental services funding.
f. Complete satisfaction surveys with caregiver clients during and/or after receiving services.
g. Work with respite service providers to identify availability, understand spending status, address billing questions, and other reasons.
h. Maintain proper documentation and system updates on caregiver clients.
i. Community education of the services to caregivers offered by COAHC.
2. Information and Referral
During assessment completion process, evaluate the need for additional resources in the home.
Provide information and refer services to client that may benefit them to encourage them to stay in their home longer.
Establish and maintain an inviting office atmosphere which makes all guests feel welcomed.
Update and maintain the appropriate client notes in CRM database in a timely manner.
Assist by updating resource guide as new services are found in the community that may benefit our clients.
3. Assessment Completion
a. Conduct client assessments both in person (office or home environment as appropriate) or on the telephone (as appropriate) as required by the funding source with a particular focus on building and maintaining rapport with clients utilizing a strengths-based approach
b. Through required assessments, be aware and identify possible co-occurring morbidities that may place the client at a higher risk of needs not being met
c. Identify clients who may need Adult Protective Services due to possible abuse, neglect, or exploitation and make a necessary referral(s) to Henderson County Adult Protective Services as appropriate
d. Based on the assessment, identify and admit clients to COAHC services who meet the criteria of current programming within the COAHC
e. Correctly complete the North Caroline DAAS 101 and client contribution agreements as required by the NC Division of Aging
f. Complete general needs assessment for each client to identify any additional assistance services, resources and supports while educating the client/caregiver on possible linkages to address identified needs outside of COAHC programming and/or make referrals as appropriate
g. Build and maintain a working knowledge of available community resources, along with ongoing resource identification and system utilization to assist clients and their caregivers with the ultimate goal of helping to connect them with appropriate services and supports
4. Perform other duties as assigned by the leadership and the Executive Director.

- Ability to supervise, train, or instruct lower-level social workers, students, or interns in the program
- Apply change, risk, and resource management principles when needed and share ideas to enhance the efficiency and productivity of procedures
- Assistance with preparation of grant reports, including but not limited to HCCBG, United Way, Hendersonville City, and Henderson County as needed
- Maintain an understanding of program budgets and required outcomes of all programs as applicable to the job position
- Ability to perform other assigned duties/tasks
POSITION REQUIREMENTS:
- Bachelor’s degree required in social work, public health, human services, psychology or a related field and a minimum of 3-5 years of clinical experience.
- Excellent communication, interpersonal and relationship building skills.
- Working knowledge of resources in Henderson County.
- Reliable transportation, a valid driver's license, and safe driving record
- Good written and oral communication skills.
- Well organized.
- Positive attitude and the ability to handle the unexpected in a calm manner.
- Must produce and maintain accurate and professional written records and reports.
- Ability to work with a diverse group of individuals.
- Proficient in planning, prioritizing, and managing work responsibilities.
- Ability to work independently and as a team player.
- Ability to work effectively with clients, co-workers, and other agencies.
