Professional case management/ Support Coordinator position. Working with a caseload of approximately 30 persons providing services to assist person with intellectual disability supports and needs.

Coordination of services to person enrolled in the Alabama Medicaid Waiver and individuals are enrolled in programs. (intellectual disability program criteria.)

Professional position that involves assessing, planning, coordination, and monitoring AL Medicad wavier Services provided to adults and children with a diagnosis of intellectual disability.

Documentation and working with persons with a diagnosis of intellectual disabilies

Training is provided for this position.

Skills required - Documentation and familiar with Microsoft excel, word and comparable projects. Use of Computer is required daily.

Pay: $32,000.00 per year
