POSITION SUMMARY

Provide client care following physician and hospice/palliative care procedures/requirements, practicing within the scope of license, certification or registration, where applicable, and as required by law and regulation.


ESSENTIAL DUTIES & RESPONSIBILITIES
  • Work to achieve performance goals, pursue progress toward individual learning plan (IDP) and demonstrate Four Seasons values and competencies
  • Provide direct client care, as attending or consulting physician, to hospice clients in a hospice facility
  • Provide medical aspects of care in collaboration with the interdisciplinary team of attending physicians, nurses, social workers, certified nursing assistants, and chaplains
  • Actively participate in weekly Interdisciplinary Team meetings for development and evaluation of the client’s plan of care
  • Participate in the coordination of the patient’s hospice care in accordance with professional standards of practice
  • Participate in the development and evaluation of the plan of care
  • Document and bill patient visits in the electronic health record (HER) timely and in accordance with Medicare guidelines and best practices
  • Contribute to patient and family counseling and education
  • Perform duties in compliance with Medicare Condition of Participation
  • Assess patients for participation in research studies
  • Maintain relationships with community physicians
  • Act as the designee of the Hospice Medical Director as required based on the needs of the agency.

  • Knowledge of HIPAA privacy rules and regulations
  • Knowledge of electronic records systems
  • Demonstrate excellent attention to detail and accuracy
  • Proficient in MS Office: Outlook, Word, Excel, etc.

MINIMUM QUALIFICATIONS:

Education:
  • Doctor of Medicine degree or Doctor of Osteopathic Medicine degree from an accredited institution
Experience:
  • 1-3 years of applicable experience in the treatment of end of life care
  • Demonstrated understanding of the Centers for Medicaid and Medicare Services requirements
  • Experience effectively collaborating with a diverse group
Licensures & Certifications: (if applicable)
  • Possess and maintain any and all applicable privileges with Mission, Pardee and/or Park Ridge hospitals
  • Valid, unrestricted, medical license to practice medicine in the state of North Carolina
  • Board Eligible or Board Certified in Hospice and Palliative Medicine, preferred
  • Valid, unrestricted, DEA registration
  • Valid driver’s license and proof of automobile insurance
