Job Description:

Description: This Palliative Care Chaplain will provide spiritual, religious, and emotional support to patients and families throughout the hospital in response to consultation with the Palliative Care Department.

Typical Duties:

  • Responsible for spiritual assessment, pastoral intervention, crisis intervention, consultation, education, and supportive services for identified Palliative Care patients, families and staff.

  • Coordinates resources and services in response to individual patient and family spiritual and emotional needs.

  • Active member of the interdisciplinary Palliative Care team, contributing to patient assessments, plans, and interventions, as well as to team goal setting.

  • Consultant within the palliative care team and participates in program development and quality improvement initiatives.

  • Provides for sacramental needs of patients either personally or through contact with appropriate clergy.

  • Maintains a contact list of the clergy of various faiths and acts as a liaison with other team members for religious needs, issues, or questions.

  • Participates in family conferences and the development of patient care plans when deemed necessary.

  • Provides for or oversees the patient's spiritual care and documents all visits, pertinent observations and spiritual care interventions in the patient's clinical record.

  • Available to prepare and conduct in-service seminars on religious or spiritual subjects/issues upon request.

  • Acts as a liaison between the palliative care team and the spiritual care & ethics department.

  • Responds to interdisciplinary consults in a timely manner.

  • Complies with call schedule in a timely manner.

  • Facilitates end-of-life planning and decedent care.

  • Provides emotional and spiritual support to staff.

  • Engages in other duties as needed to further the mission of the Palliative Care Department.


Qualifications:


    Required Education and Experience:
  • Master's degree (MA, MS, MBA) or equivalent in Divinity; Theology; Pastoral Care; Religious Education from an accredited college or university
  • Minimum 4 units of Clinical Pastoral Education
  • Three years of previous experience providing spiritual care in a healthcare environment, including care for the dying and their families
  • Minimum one year experience in hospice or palliative care chaplaincy service
  • Preferred Education and Experience:
  • Five years of previous experience providing spiritual care in a healthcare environment, including care for the dying and their families
  • Preferred Licensure/Certification/Specialized Training:
  • Eligible for board certification with the Association of Professional Chaplains, the National Association of Catholic Chaplains, the Spiritual Care Association, or the National Association of Jewish Chaplains.
  • Board-Certified Chaplain – Palliative Care and Hospice Advanced Certification.
  • Bilingual (English/Spanish)
