POSITION PURPOSE:
Assess all prenatal patients and other assigned to identify social, economic, emotional, behavioral and other issues that may negatively affect the patient.
POSITION REQUIREMENTS / QUALIFICATIONS:
Education/Experience: Bachelor’s Degree in Social Work with minimum of two (2) years clinical experience. Knowledge of Laws, rules re: Public Assistance, Medical Care by Federal, State and Count.
Licensure / Certification:
Maintain current CPR certification from the American Heart Association.
Bi-lingual Skills
Licensed Clinical Social Worker preferred.
Must have an active Florida Driver’s License.
Skills / Ability:
Sensitivity in working with diverse cultural groups. Good communication skills and interpersonal relationship.
POSITION RESPONSIBILITIES (THIS IS A NON-EXEMPT POSITION)
Complete a comprehensive initial assessment of clients to determine client’s physical, social, emotional, medical and environmental needs/problems and establish desired outcomes.
Develop a CARE plan in conjunction with the client and family members which address the problem identified in the assessment process.
Arranges for services needed by the clients.
Follow-up on contacts made for services.
Maintains regular contact with clients to ensure that their needs are met.
Conduct regular reassessment of the client and revise the CARE plan and service arrangements accordingly.
Documentation of all information services, outcomes and actions taken on behalf of the client.
Participates in staff conference with other program staff to assure that client’s are being appropriately met.
Attend scheduled in-service training sessions.
Respond to emergency situation, which require immediate intervention.
Perform home and hospital visits to ensure continuity of care.
Complete all administration and statistical reports as required.
Participation in Performance Improvement Programs and Peer Review as required.
Involved in the active monthly case goal of 50-60 clients unduplicated prenatal patients and their newborn up to age 2.
Maintains 80% productivity monthly
Reports to work on time and ready to work with minimal absenteeism.
Completes B&E (billing and encounter) forms within the same day of intervention.
Provides accurate and timely documentation in patient charts within the same day of intervention.
Adheres to Confidentiality Policies and Procedures / HIPAA Regulations.
Perform other duties as assigned.
