Spring Hills Post Acute Matawan, a 130-bed Skilled Nursing Facility, is seeking a part-time Social Worker. This position will be for 20 hours per week. Reporting to the Director of Social Services, this position will assist with social service intakes for new admissions, discharge planning, care plan conferences, completion of applicable MDS sections, and other duties as assigned by the Director.
