Our Early Intervention program is seeking a Service Coordinator to provide service coordination to children in home-based, community, or childcare based settings. If you aren’t afraid to have fun with your job and will bring enthusiasm, dedication, and spirit; we want YOU to join our Team!

As a Service Coordinator, you will be responsible for:

  • Protecting the rights and dignities of individuals with developmental disabilities and extending these rights and dignities to family members or guardians
  • Monitoring and assisting the child and family in programmatic interventions including but not limited to gaining access to needed medical, social, educational, and other developmental/therapeutic services
  • Functioning as a member of a trans-disciplinary team serving infants and toddlers and obtaining consultation outside own expertise when necessary
  • Performing service coordination, evaluation and assessment, IFSP development, and delivery of EI services requiring qualified personnel
  • Participating in the treatment; planning process and monitoring progress in meeting the goals and objectives of the IFSP (Individual Family Service Plan)
  • Documenting all activities in accordance with requirements necessary for filling, agency policies, or Early Intervention regulations
  • Providing family training and counseling in a home-based, community, or childcare based setting to achieve goals in all necessary areas
  • Providing direct and /or indirect Early Intervention Services as qualified
  • Mentoring Service Coordinator I personnel and Early Interventionists
  • Performing other duties as directed by the supervisor related to the organization’s mission, goals, and operations

Education/Requirements:

  • Bachelor’s degree required with Certification/Licensure in Early Childhood Education, Child Development, Early Childhood Special Education, Social Work, Psychology, Communication Disorders, Nutrition, or a related Early Intervention field
  • Master’s Degree preferred
  • 2-5 years of experience providing interventions to infants and toddlers (birth to age three) in a family-focused, home-based setting; experience in the trans-disciplinary provision of support
  • Advanced knowledge in one or more special treatment areas such as PDD/autism, second language and/or experience working with minority, racial, ethnic, and immigrant populations preferred
  • Must be willing to obtain and maintain pediatric CPR certification
  • A minimum of 12 hours per year of training with a focus on working with young children with disabilities and their families as well as completion of the Introduction to EI course within six months of the date of employment is required
  • Must have a valid driver’s license, safe driving record, and proof of auto insurance
