HSI is a nonprofit that has been serving vulnerable New Yorkers for over 35 years. Our mission is to end the cycle of homelessness permanently by providing safe, affordable, and empowering environments for our tenants to live, combined with comprehensive services and other access to care that helps them truly thrive. HSI creates communities where our tenants want to live, and our staff love to work. Our 100-unit Scatter Site Program consists of 60 units in upper Manhattan and 40 units in the Bronx in privately owned apartment buildings that serve 75 single individuals and 25 family households impacted by HIV/AIDS. HSI’s Scatter Site team consists of a Program Manager, Assistant Program Manager, five Case Managers, and a 2-person Maintenance department.
QUALIFICATIONS: High School required; BA/BS in social work, human services or health discipline preferred. Prior case/care management experience, especially in a supportive housing program, with homeless persons, the mentally ill, substance users, and persons living with HIV/AIDS strongly desired.
LOCATION: The Scatter Site team works out of HSI’s main office, located at 243 West 30th Street, NY NY 10001; Tenant home visits require travel (compensated) to Upper Manhattan and the Bronx; FLEX SCHEDULE: work one day a week remotely from home.
Duties and Responsibilities:
The Scatter Site Case Manager will maintain a caseload of 20 households (mix of individuals and families) wherein they will provide:
- provide direct tenant services, including, but not limited to, case management and overall care coordination through regular contact with assigned tenants via phone, home visits, office meetings, and escorts
- provide emotional support to tenants from a harm reduction, tenant centered, strengths based and housing first perspective
- work collaboratively with tenants to assist with identifying and working toward their personal goals (educational, vocational, health, familial or other)
- provide tenants the necessary assistance to obtain and retain all appropriate public benefits and entitlements, and provide escort and advocacy services as necessary
- develop and maintain working knowledge of and regular contact with health care and social services systems/agencies to ensure tenant access to, referral and placement in all services, such as HIV/health care, mental health, and substance use/dependence treatment
- maintain accurate and proper record documentation, such as up-to-date case management notes, service plans, psycho-socials, and other summaries
- alert Program Manager and Assistant Program Manager regarding tenant emergencies and other immediate needs and assist in efforts to promptly deliver proper services
- educate and support tenants’ skills to aid in independent living, such as in the maintenance of apartments, household budgeting, improved interpersonal relationships, self-care needs, and moving into/out of apartments
- work in close cooperation with the Program Manager and Maintenance staff to ensure the proper physical upkeep of tenant apartments and the prompt payment of utilities
- obtain regular (supervision with the Program Manager (Assistant Program Manager in Program Manager’s absence) to review tenant issues/concerns and service needs, among other matters
- participate in weekly staff and other meetings, such as in-service training and staff development and orientation activities
- provide the Program Manager and Assistant Program Manager with input and tenant information for the weekly, monthly, quarterly and other reporting to HASA, among other agencies as needed
- work collaboratively with the Scatter Site team to design and implement program and service enhancements
- plan and facilitate tenant support groups on a variety of topics related to their needs and interests
- help run holiday events and escort tenants to program outings; attend tenant advisory board meetings as needed
- perform all other duties as may be assigned by the Program Manager or Assistant Program Manager
Type: Full-time Salary: $50,000 to $52,000 per year

Experience: Minimum 2 years’ experience preferred. Prior case/care management experience, especially in a supportive housing program, with homeless persons, the mentally ill, substance users, and persons living with HIV/AIDS strongly desired.
Education: High School required; BA/BS in social work, human services or health discipline preferred COVID-19 considerations: As per the CDC Guidelines: Staff are now required to be fully vaccinated upon 1st date of hire and possess the ability to provide documentation.
