JOB DESCRIPTION
JOB SUMMARY:
The RN Clinical Supervisor is a Registered Nurse who will be responsible for managing clinical services in the home care agency. Primary responsibility will be to evaluate clinical practices of specified disciplines through the review of clinical records to ensure that standards of care are met, and quality services are provided in accordance with PARC Home Care policy and procedures, local, State, and Federal guidelines. Occasionally performs emergent patient visits as needed in the event of staff shortages. This individual will serve as a backup to the Clinical Director. QUALIFICATIONS:
1. Graduate of an approved school of professional nursing and currently licensed in the state(s) in which practicing.
2. Two (2) years of clinical home care experience preferred.
3. Acceptance of philosophy and goals of this Agency.
4. Knowledge of state and federal home health agency regulations/Conditions of Participation and Compliance standards and regulations.
5. Knowledge of Clinical policies and independent judgement.
6. Oasis experience preferred.
7. Good organization, communication, human relations skills, ability to multitask and reliable transportation.
8. Ability to deal effectively with high levels of stress.

RESPONSIBILITIES:
1. Evaluates clinical practices of specified disciplines through the review of clinical records to
ensure that standards of care are met, and quality services are provided in accordance with PARC Home Cares policies and procedures, local, State, and Federal guidelines. This
includes, but will not be limited to:
a. Reviews visit notes to ensure that plan of care is being properly executed, planned
visit frequency is being adhered to, and care goals and objectives being assessed,
measured, and accomplished on an on-going basis.
b. Reviews patient charts to ensure that wound care orders are being properly executed.
c. Reviews patient charts to ensure that interdisciplinary care
coordination/communication is taking place.
d. Reviews intervention by different disciplines and minimizes duplication of effort and
overlapping of services.
e. Determines adequacy of proposed visit frequencies given patient’s acuity and plan of
care.
f. Review OASIS documents for accuracy and completeness prior to locking and
submitting data electronically.
g. Assist with contacting clinician with outstanding documentation to facilitate finalizing of claims.

2. Supports Intake/Scheduling departments with coordinating staffing patterns to optimize utilization of clinical staff while at the same time meeting patient needs and the needs of the interdisciplinary geographical clinical teams. This includes, but will not be limited to:

a. Assists Intake with reading and interpret clinical documents, such as discharge plans,

physician progress notes, and orders.

3. Assists the Clinical Director with facilitating continuing staff education to meet state,

federal and PARC Home Care requirements. This includes, but will not be limited to:

a. Educate and mentor clinicians one on one (in person or over the phone) regarding

concerns, questions, accuracy of OASIS data, and other related documents, that are

being reviewed.

b. Assists with coordination of orientation and training for all Clinical new hires

c. Assists with providing ongoing training and education opportunities for Clinical Staff

d. Assists with clinical orientation duties including documentation and quality

improvement for all new clinical employees.

e. Assist with staff competency, job performance, and compliance to standards of care,

best practice, and PARC Home Care policies and procedures.

4. Participates in Performance Improvement activities including but not limited to:

a. Clinical record review, quality indicator monitoring, and quality management.

b. Actively participates with company committees (QAPI) to achieve desired outcomes

including optimization of OBQI scores, refinement of operating processes, updating

existing policies and procedures, enhancing the use of support staff, and proper

management of supply utilization.

5. Manages medical supply requisitions.

6. Manage emergent patient visits as needed for patient coverage in the event of staff shortages.

7. Other duties as assigned by the Clinical Director

Services/Administrator.

WORKING ENVIRONMENT:

Works indoors in Agency office and patient homes and travels to/from patient homes.

Ability to perform the following tasks if necessary:

  • Ability to participate in physical activity.
  • Ability to work for extended period of time while standing and being involved in physical

activity.

  • Moderate lifting.
  • Ability to do extensive bending, lifting, and standing on a regular basis.
