We are currently seeking an energetic Registered Nurse/Clinical Manager to assist the agency with the coordination of home health services to patients in their place of residence in accordance with physician orders. The RN Clinical Manager is under the supervision of the Director of Nurses. The RN Clinical Manager is available all times during operating hours and participates in all activities relevant to professional services furnished, including the management of patient care. Must be able to manage agency activities with DON not present. The RN Clinical Manager will participate in developing standards which ensure safe and therapeutically effective service to patients and families. Ability to provide oversight in a positive manner to team members in order to meet the needs of the agency.

Qualifications:

- Graduate of an approves school of professional nursing and currently licensed in the state of Louisiana

- Three to five years of nursing experience

- At least two years of home health experience

- Acceptance of vision, philosophy and goals of Agency

- Ability to problem-solve when conflicts / issues arise

- Ability to exercise initiative and independent judgment

- Ability to work with individuals to enlist cooperation of many people to perform/achieve a common goal

- Knowledge of Agency, DHH Standards and CMS Conditions of Participation

-Participates in planning for he orientation of new employees.

-Serves as a role model for all colleagues and serves as a resource person to all employees

-Attends pertinent continuing education programs other than routine in-services and shares information with staff. Stays abreast of new developments in the home health arena

-Responsible for assisting with all staffing and visit assignments and have a understanding for meeting the needs of the agency

-Addresses, corrects and resolves patient concerns

-Participates and assists with any special projects taken on by behalf of the Director of Nursing to meet the needs of the agency.

-Experience in Homecare Homebase a PLUS+

Pay: $65,000.00 - $88,000.00 per year
