This position is primarily located at the assigned, local branch office(s).
POSITION OBJECTIVE
The nursing licensed professional administers skilled care to patients requiring intermittent professional services. This position will teach the patient, family, and other members of the health care team. These services are performed in accordance with the physician’s orders and the established plan of care.
EDUCATION, EXPERIENCE, QUALIFICATIONS
Education and Experience (ESSENTIAL):
Must be a graduate of an approved school of clinical education. Must be licensed in the state where they currently practice. Must have at least one year experience as a licensed professional.
Education and Experience (DESIRED):
Experience as a licensed professional may be deferred with a review and approval from the regional vice president for operations or regional director.
Medicare home health or hospice experience is preferred.
Qualifications:
Must be organized and detail oriented. Must be able to communicate effectively both orally and in writing.
Must understand the issues related to the delivery of home health or hospice care and be able to problem- solve effectively. Must be capable of prioritizing and handling multiple tasks of critical importance simultaneously. Must function well in atmosphere of stress and interruption.

- Maintain dependable transportation in good working condition
- Must be able to safely drive an automobile in all types of weather conditions
- Must possess CPR certification for the healthcare provider

SPECIFIC DUTIES AND RESPONSIBILITIES
The duties and responsibilities of this job are outlined below.
Administrative:
- Complete all appropriate documentation and submit timely.
- Participate in staff development meetings and in-services.
- Participate in case manager and nursing meetings.
- Participate in the planning, operation, and evaluation of professional services.
Licensed Professional (RN) Home Health Prepare for and participate in the weekly case conference meetings, ensuring you have adequate knowledge to decide if patient’s care should be continued into another health care episode. Follow company policy and direction timely for software and mobile device updates.

- Obtain continuing education to improve care, broaden knowledge of changes in health care delivery, and to meet any licensure requirements.
- Conduct supervisory visits in a timely manner as required for licensed practical or vocational nurses to ensure appropriate service provision and compliance to all state and federal regulations.
- Assist with conducting on-site supervisory visits, as directed.
- Participate in on-call rotation to perform after hours and weekend evaluations, visits, as directed; ensure appropriate and safe response to after hours telephone calls.
- Complete all other duties as assigned.Clinical:
- Perform patient visits in a timely, professional, and appropriate manner per standards of care. Ensure in-home visit time fully meets the needs of the patient.
- Coordinate total patient care including interdisciplinary communication with other health care providers, office, and physician.
- Contact physician regarding patient changes; receive and write verbal orders.
- Review and verify schedules weekly to ensure accuracy.
- Update the plan of treatment, as needed. Notify the physician and branch staff of significant changes in the patient’s status.
- Report the need for add-on services to the appropriate branch staff.
- Guide and instruct patient and their families in prescribed activities that are directed toward improving independence and functionality.
- Cooperate and assist other health care providers to enhance continuity of care.
- Review and update the medication profile; assess patient compliance, understanding of medication regime, contraindications, and side effects.
- Complete admission, re-certification, post hospital, and follow-up assessment procedures according to established procedures.
- Plan, coordinate, and perform all patient functional re-assessment visits per Medicare regulations.
- Conduct discharge assessments and ensure appropriate discharge teaching; retrieve medical record data from patient home if discharging the patient from services.
- Complete all OASIS documentation accurately, completely, and timely.
- Ensure lab tests are obtained, stored, and transported according to acceptable laboratory practice and occupational safety and health administration guidelines.
- Review and evaluate client lab results and ensure appropriate physician notification.
- Ensure that missed patient visits are rescheduled in the same Medicare week in order to adhere to the patient’s POC.

Human Resources:
- Provide requested documentation timely to keep personnel file current and in good standing.
- Comply with activities associated with the management of workplace health and safety.
- Identify and report health and safety risks, accidents, incidents, injuries, and property damage at the workplace in a timely manner.
- Maintain a professional relationship with all workers, interns, volunteers at all times.
- Adhere to Choice Health at Home dress code at all times.
Licensed Professional (RN) Home Health Properly request and utilize appropriate personal protective equipment, as applicable.

- Be knowledgeable and comply with HIPAA privacy and compliance programs.
