Job description
As a REMOTE Supportive Care Palliative Registered Nurse you assist some of the most complex patient population in navigating the health care system and making it work for them. You are able to support and look at a patient holistically, helping them and their whole care team, navigate the health care system. You are their advocate and get 1:1 time to assist in impacting the patient experience.
The Supportive Care Palliative Registered Nurse assesses, anticipates and addresses the physical, psychological, social, and spiritual needs of individuals with a life-limiting condition who could benefit from supportive care.

Role and Responsibilities for Supportive Care Palliative Registered Nurse:
- Coordinate with ROM, CQA, and RNs to review patient cases and identify clinical judgement escalations
- Support Patients with potentially declining condition to educate on treatment options, ensure patients care aligned with their wishes, and navigate to appropriate resources.
- Assist patients with identifying their goals, values, preferences, and develop care plan that aligns with patient wishes.
- Work with patients to develop and/or update advance care plans.
- Facilitate patient/family communication
- Serve as a role model and coach to help peers facilitate ACP and EOL conversations
- Provide comfort case management guidance to IKC RNs for patients where palliative care is not available or when patient is not ready to stop dialysis but would like to pursue comfort care
- Serve as a resource for state-specific documentation and regulation requirements for ACPs
- Coordinate care between patients care team (Neph, PCP, dialysis clinic, supportive care resources, etc.)
- Develop relationships with supportive care providers
Qualifications for Supportive Care Palliative Registered Nurse:
- A.D.N degree from accredited school of nursing required; B.S.N preferred
- Considerable experience facilitating advance care planning and end-of-life discussions; has the ability to modify communication style to facilitate conversations and meet patient’s needs
- Current Registered Nurse (RN) license in the state of practice required
- Continuing education credits maintained as required by state of practice required
- Minimum of three (3) years’ experience in Case Management required
- Minimum of two years working in palliative or hospice care setting
- Demonstrated knowledge and understanding of data and managing to clinical, financial, and patient satisfaction outcomes
- Experience and passion for educating patients
- Current CPR certification required
- Ability to adapt to new/changing technology
- Current driver’s license in state employed with positive driving record and able to meet requirements of insurance coverage required
- Basic computer skills and proficiency in MS Word and Outlook required
