Registered Nurse (RN)

This Registered Nurse (RN) position requires no nights, weekends, holidays, or on-call. Enjoy a work/life balance with this full-time 35-hour, hybrid remote, work week and the opportunity for a four-day work week after six months. Bristol Elder Services, Inc., located in Fall River, MA, is a progressive, private, non-profit agency that supports the independence and dignity of the elderly and persons with disabilities in the community.

RN Responsibilities include:

· Assessing and screening clients for clinical eligibility for home care services or nursing facility placement, as well as other programs in the community

· Participating in developing community-based service plans with consumers and family members

· Active RN member of interdisciplinary team meetings, coordinates with vendor RNs, and develops personal care plans geared at keeping elders safe in their homes

· Conducting long-term and short-term nursing facility screenings for MassHealth members and collaborating with others as needed to facilitate safe discharges when appropriate

Requirements include:

· BSN or ASN and at least one (1) year clinical experience and/or nursing in a community or long-term care setting; OR a Diploma RN and two (2) additional years of community-based nursing experience or long-term care direct service experience

· MA RN license

· Valid driver’s license

· Reliable transportation

· Great assessment and verbal skills needed

· Strong attention to detail

· Experience using computers is a must- proficient in MS office and EMR.
