WESTARM Home Health is looking for a RN to provide home health services on a full-time basis in the Lower Burrell, New Kensington, Leechburg, Apollo, Plum and surrounding areas. This is a great opportunity for a RN that enjoys connecting with patients and having independence in their work while working for a small and local company. You will be able to make your daily schedule. Occasional weekend rotation required. Extra pay for weekend visits.

PRN (pay per visit) employees are compensated at $50/regular visit and $85/evaluation visit on weekdays.

Responsibilities:

Primary functions are to plan, develop, implement, evaluate, organize and direct home care services with the interdisciplinary team including physician, patient/family, and administer skilled nursing care for clients in their place of residence and assume the responsibility for the total provision of care.

Essential Duties and Responsibilities:

• Assesses home health patients to identify the physical, psychosocial, and environmental needs as evidenced by documentation, clinical record, case conference, team report, and evaluations
• Case manages and provides clinical oversight and direction to the interdisciplinary team, physician, and family
• Implements/develops/documents the plan of care and revises as necessary
• Assures continuity of quality patient care delivered with appropriate documentation
• Monitors assigned cases to ensure compliance with requirements of third-party payors
• Provides effective communication to patient/family, team members, physicians, and other health care professionals
• Adheres to all company policies and procedures.

Minimum Qualifications:

• Graduate of an accredited college/university, associate, diploma, or baccalaureate school of nursing
• Current State License as a registered nurse or licensed practical nurse
• Current Driver's License
• Current CPR, TB screen and Hepatitis consent
• Two years of experience as a registered nurse in a clinical care setting preferred, home health experience preferred.

Job Type: Full-time

Pay: $40.00 - $60.00 per hour
