RN Office Based at Giving Home Health Care, Las Vegas office.

Patient needs range from monthly hours to 24-hour care.

Please apply if you have previous home health or skilled nursing facility experience and have managed the admission / discharge process, nursing schedules and exposure to regulatory requirements/audits.
This is a full-time position. Occasional home visits for supervisory visits, assessments, and training. Most of your time will be in the office working with the Directors of Nursing, caretaker staff, community advocates, and case managers.
Your writing and assessment skills must be sharp as you will be doing some medical and clinical writing and review. Your communication skills must be effective, warm, friendly but persuasive because in addition to your nursing staff, you will work with many doctor's offices and clinics that we depend on to keep services authorized. Hence, your written and verbal presentation must be humbly confident without being pushy.

Summary of Requirements
- Must be an RN with active license in Nevada.
- BLS
- Humility with those you supervise.
- Excellent written and verbal communication skills.
- Strong computer skills and experience with practice management software.
- Experience making employment as well as HR management decisions.
- Able to work 40 to 45 hours a week when needed.
- Desire to make a difference in the lives of some amazing people.
Job Responsibilities:
- Work closely with care team to identify high-risk, complex patient cases; manage care, including referral management, care planning, post-hospitalization planning and coordinating community-based care.
- Meeting with care team to discuss plans based on patients’ progress and input from physicians and other medical providers.
- Conduct individualized clinical evaluations of patients, their health needs, and concerns; develop personalized action plans, provide education, and issue referrals for care management programs; monitor patients’ progress in meeting established goals and modify care plans accordingly. Guide, intervene and advocate on behalf of patients, their caregivers and/or families about navigating and comprehending the healthcare system.
- Assist in reviewing patient's clinical diagnosis, medications, procedures, and clinical course.
- Educate care team members and patients on benefits, services, and other resources.
- Assist in developing a cohesive and strong team-oriented relationship with physicians, nurses, and other healthcare professionals.
- Assists in the formulation, revision, implementation, and evaluation of policies and procedures, as well as strategic goals and objectives.
- Ensures quality management within the agency through participation in monthly case management notes audits and outcome measures, in collaboration with other supervisors.
- Review and sign case management notes
- Conducts new patient nursing assessments.
- Assists in the implementation of current best practice models.
- Coaches team members on how to develop proactive, strength-based, family centered, treatment plans for patients receiving services, in collaboration with staff, Manager, Clinical Director, and other consultants.
- Participates in team meetings and management team meetings, as appropriate.
- Assist with training and supervision of field staff when needed.
- Work with administrative and supervisory personnel regarding home care visits to assure high quality and proper follow-up patient care about patient needs.
