Minimum Qualifications:
The position is in a Family Practice clinic in Kaneohe Bay, HI. It is full time Monday - Friday with a full benefits package.
Degree: Possess a Baccalaureate Degree in Nursing, a Graduate Diploma in Nursing, or an Associate Degree in Nursing.
Education: Graduate from a college or university accredited by National League for Nursing Accrediting Commission (NLNAC), or The Commission on Collegiate Nursing Education (CCNE).
Experience: Possess a minimum of 2 years of experience within the preceding 3 years in an ambulatory care clinic setting.
License Current, full, active, and unrestricted license to practice as a Registered Nurse as required in the TO.
Core Duties:
- Coordinate patient care in collaboration with a wide array of healthcare professionals. Facilitate the achievement of optimal outcomes in relation to clinical care, quality and cost effectiveness.
- Ensure compliance with standards of care and practice in accordance with all established policies, procedures, and guidelines used in the MTF.
- Perform physical exam and health histories.
- Provide health promotions, counseling, and education.
- Administer medications, wound care, and numerous other personalized interventions.
- Direct and supervise care provide by other healthcare professionals.
- Accountable for making patient care assignments based on the scope of practice and skill level of assigned personnel.
- Recognize adverse signs and symptoms and quickly react in emergency situations.
- Communicate and collaborate with a diverse group of people for the purpose of informing the healthcare team of plans/actions, for teaching/education to benefit the patient/family and organization.
- Make referral appointments and arrange specialty care as appropriate.
- Perform nursing services identified in the TO.
- Conduct research in support of improved practice and patient outcomes.
- Prior to providing services the RN will complete a unit orientation and competency assessment. The RN must be deemed fully competent by their immediate supervisor no later than 60 days after commencement of services.
- Provide comprehensive nursing care for all ages and families in the Medical Home Port (MHP) Team utilizing evidence-based clinical practice guidelines and engage patients actively in their health care.
- Collaborate and integrate with members of the MHP Team, other departments, and directors to achieve optimal care for the patient in the MHP model.
- Actively participate in MHP Team huddles. Proactively assist the team’s identification of diagnostic procedures required during schedule scrub. Assist with the identification of patient education needs and review plan with team members for coordination of educational needs.
- Coordinate the screening of enrolled patients to identify patients needing preventative services and education based on their specific health care needs including, but not limited to, health and wellness topics, medical conditions, and medical procedures.
- Engage other MHP Team members to assist in improving documentation of the episode of care using MHP Clinic-approved templates and the initial template (i.e. TriService Workflow/COMPASS).
- Use secure messaging in accordance with departmental or command standard operating procedures.
- No blood products or TPN are administered at this clinic.
GENERAL DUTIES. Contractor personnel shall perform a full range of duties consistent with their labor category, education, training, experience, clinical privileges (as applicable), and assigned position. General duties that apply to all personnel or across certain labor categories are given below.
Maintain a level of productivity comparable with that of other individuals performing similar services.
- Participate in peer review and performance improvement activities.
- Practice aseptic techniques as necessary. Comply with infection control guidelines to include the proper handling, storage, and disposal of infectious wastes, and the use of universal precautions to prevent the spread of infection.
- Function with an awareness and application of safety procedures.
- Perform efficiently in emergency patient situations following established protocols, remaining calm, informing appropriate persons, and documenting events. Anticipate potential problems/emergencies and make appropriate interventions. Notify supervisor, director, or other designated person regarding problems that the HCW is unable to manage.
- Apply an awareness of legal issues in all aspects of patient care and strive to manage situations in a reduced risk manner.
- Participate in the implementation of the Family Advocacy Program as directed. Participation shall include, but not be limited to, appropriate medical examination, documentation, and reporting.
- Exercise awareness and sensitivity to patient/significant others' rights, as identified within the MTF.
- Maintain an awareness of responsibility and accountability for own professional practice.
- Participate in continuing education to meet own professional growth.
- Attend and/or comply with all annual training classes required by the Command, to include but not limited to online annual training provided by the MTF: disaster training, infection control, Sexual Harassment, Bloodborne Pathogens, Fire and Safety, Chemical, Biological, Radiological, Nuclear and Explosives (CBRNE), and all other required training.
- Actively participate in the command’s Performance Improvement Program. Participate in meetings to review and evaluate the care provided to patients, identify opportunities to improve the care delivered, and recommend corrective action when problems exist.
- Participate in the provision of in-service training to clinic staff members. Provide training and/or direction as applicable to supporting government employees (e.g., hospital corpsmen, students, etc.).
- Attend and participate in various meetings as directed.
- Perform timely, accurate, and concise documentation of patient care.
- Operate and manipulate automated systems such as CHCS, AHLTA, ADS, Essentris, MHS Genesis and Clinical Information System (CIS), participate in clinical staff Performance Improvement (PI) and Risk Management (RM) functions, as prescribed by the Commander. Maintain DOD email account as directed. The HCW shall be responsive to all email and voicemail communications.
- Exercise appropriate delegation of tasks and duties in the clinical direction and coordination of health care team members, patient care, and clinic activities.
- Maintain documentation of all treatment provided in accordance with clinic directives, and prepare such records and reports as may be required. All records and reports must be legible. Abbreviations must be only those listed in local instructions.
Physical Demands: The contract HCPs should be aware that the position requires constant standing, walking, lifting and bending. In addition, the employee may be exposed to highly contagious/infections disease, radiation from portable x-ray, and continuous fluoroscopy equipment.
Work Environment: Work is performed with potential constant exposure to various diseases, emotionally disturbed and disruptive personalities. There is exposure to electrical equipment, portable x-ray radiation, pathogens and blood and body fluids and airborne pathogens. Personal protective equipment will be worn in accordance with established local policy.
Hours/Schedules: Services shall be provided Monday through Friday, between the hours of 0600 and 1800. Shifts will normally be scheduled for an 8.5 to 9-hour period, to include an uncompensated 30 to 60-minute lunch break, respectively. In no instance will the HCW be required to provide services in excess of 80 hours per two-week period.
