Top Managed Care company is hiring an Registered Nurse to work as a Case Manager/Healthcare Consultant in the greater Richmond, VA area!

Must reside in the Greater Richmond/Petersburg VA area

Position requires Face to Face assessments and will be Work from home when not in the field.

Nurse Case Manager is responsible for telephonically and/or face to face assessing, planning, implementing, and coordinating all case management activities with members to evaluate the medical needs of the member to facilitate the member’s overall wellness. Develops a proactive course of action to address issues presented to enhance the short and long- term outcomes as well as opportunities to enhance a member’s overall wellness through integration. Services strategies policies and programs are comprised of network management and clinical coverage policies.

• Provide comprehensive healthcare management services to facilitate appropriate healthcare treatment, effectivelymanage healthcare costs and improve healthcare program/operational efficiency involving clinical issues
• Determines medical necessity/ appropriateness Facilitates optimal outcomes
• Identifies and follow through with continuous quality/ compliance opportunities . May also include identification of aberrance's and initiation of corrective action
• Educates/empowers customers to ensure compliance, satisfaction and promote patient advocacy
• Optimize total costs Implementation and evaluation of policy based on usage and program directives
• Educate/empower colleagues at all levels to enable decision making at most appropriate level

Schedule: Mon-Fri 8am-5pm

REQUIREMENTS:

• Active and Current Virginia RN License
• Associate or Bachelor's in nursing
• Case management and/or discharge planning experience preferred
• All applicants must be Vaccinated for Covid-19 to be considered.
• 2 Years of RN experience
• BLS/CPR
• VA Driver's license in good standing
• Valid Auto insurance

Job Types: Full-time, Contract

Pay: $34.00 - $35.94 per hour
