Job description
· Position responsible for both indirect and direct patient care using professional nursing skills and knowledge.
· Analyzes health status of patients in determining nursing care needs, identifying goals and patient nursing care plans.
· Responsible for ensuring quality care services are delivered to the patients.
· RN has oversight of her caseload and all RN, LPNS that work with the patients on her caseload. This is placed into territories.
· Position requires second-tier problem solving and compliant resolution while maintaining high standards of ethics and integrity, including good communication, confidentiality, compliance with laws and company policies, and professional conduct.
· RN Case Manager reports to the IDD, Home Health and Nursing Executive Supervisor
Primary Responsibilities:
· Evaluates the effectiveness of the quality of nursing care provided.
· Participate in the development of an appropriate individualized care plan based on age, cultural and spiritual values.
