HeartShare St. Vincent's is looking for a Registered Nurse to do medical assessments for medically fragile children who are clients in the Special Medical Prevention program.
Responsibilities:

• Face to face home visits are required for all new clients and all high risk patients need to be seen consistently to assess that treatment is being followed.
• Documentation of visits required to be written in CONNECTIONS and all contacts recorded in PROMIS (both NY State child welfare programs).
• Must be willing to do home visits throughout all 5 boroughs of NYC.
• Office hours twice a week for meetings, conferences and to document contacts.
• Participate in trainings, staff meetings, case consultations and supervision required.

Qualifications:*

• Must be a Registered Nurse (RN). Can have an Associate Degree or Bachelor's.
• Knowledge of Child Welfare helpful but not required.
• Team approach to reduce risk to children who are medically fragile.
• New graduates welcomed to apply.
