Community First Health Centersis currently looking for an exceptional Referral Specialistto join our team at our New Havenmedical facility. The ideal candidate for this role will be enthusiastic, motivated and a team player.

Under the supervision of the Medical Clinic Practice Manager, you'll support the medical clinic operations by maintaining patient electronic medical records and physical charts as directed.

This position will be located out of our New Haven, MI medical facility located at 58144 Gratiot Avenue.

Daily responsibilities of our Referral Specialist may include:

• Promotes and maintains awareness of agency programs, services and activities
• Maintains communication between medical staff, patient(s) and insurer(s)
• Participates in Quality Improvement activities as needed/directed
• Assists with patient referral(s) based on requirements of insurance and HMO companies
• Assist patient(s) with setting up referral(s) and diagnostic testing; tracking of all referrals and diagnostic testing
• Complete all prior authorization(s) needed for referral(s) and diagnostic testing; perform prior authorization(s) for patient medication as needed/directed
• Attend meetings, seminars, workshops and conferences, as needed and approved for professional development and on behalf of Community First Health Centers
• Assist in the development and generation of referral related or other requested reports
• Other duties as assigned
