Position Summary Statement:
The purpose of Casa’s Reentry Program is to reduce relapse and recidivism among Latino reentering citizens living with co-occurring substance use and mental health disorders (CODs) and help them develop skills to build meaningful, purposeful lives in the community. The Reentry Specialist will use assertive outreach strategies to develop and strengthen systems and linkages to enhance and expand access to a recovery-oriented continuum of care and coordinate with correctional facilities to transition participants to Casa’s fully integrated, culturally-competent, reentry-oriented Treatment and Recovery Support Services to build the Recovery Capital (internal and external resources) necessary to lay the groundwork for change, support sustained recovery, and facilitate meaningful participation in the community. The Reentry Specialist will help clients to make the transition from life in incarceration to life in the community, through the coordination of wrap-around recovery services. The RES will be familiar with and implement services using Motivational Interviewing and Motivational Counseling Techniques, as well as being familiar with both strengths-based and trauma-informed approaches to treatment.
As part of the Casa Esperanza, Inc. integrated care team, the Reentry Specialist provides services in a fully integrated, holistic, and collaborative manner using a team-treatment approach, to ensure the coordination of all client services, both across Casa programs as well as with external providers. This integrated model aims to facilitate clear communication amongst all providers and eliminate duplication of efforts or conflicting approaches to care; details necessary follow up and assignments; and ensure-wrap around support services are in place to maximize treatment effectiveness and clinical outcomes.
Responsibilities
- Conduct assertive outreach to correctional facilities, parole/probation boards, and other organizations working with returning citizens.
- Work with Project Director and Marketing Staff to create a Latino Reentry Network (LRN) to develop a comprehensive, multi-agency approach to improving reentry services focused on the unique needs of multiply-diagnosed Latino reentering citizens.
- Support individuals in successfully transitioning to residential treatment
- Conduct pre-release assessments at assigned correctional facilities, including screenings for co-occurring disorders, overdose risk, and recidivism risk of potential participants in corrections.
- Complete RNR assessment for individuals while incarcerated to determine target needs and appropriate programming.
- Support corrections case managers in developing comprehensive transition plans for participants that addresses logistical and emotional transition needs and recovery-oriented goals.
- Coordinate wrap-around recovery services for clients, including supported referrals to integrated primary healthcare services on-site and transportation as needed for individuals coming to residential treatment from corrections.
- Consult with parole officers, providing all necessary information for the parole supervision plan.
- Consults with specialty Courts re: patients, and networks with colleagues (parole, probation, community corrections, etc.) across the correctional continuum of care. (added)
- Conduct urine screening for clients, as needed
- Document all client progress and manage client files.
- Complete baseline and follow up questionnaires and assessments, as needed.
- Participate in weekly clinical team meeting to address client needs.
- Participate in program development with the clinical team.
- Attend grantee meetings and conferences as appropriate or required.
- Facilitates Recovery/Reentry Groups on Casa campus (i.e., Double Trouble, Thinking for a Change, etc.)
- Facilitate reentry groups inside correctional facilities as scheduled.
Qualifications
- Associate degree required, Bachelor’s degree preferred
- Bilingual Spanish/English, with intimate knowledge of the cultural social, economic, health, and substance abuse factors related to reentry.
- Ability to obtain clearance for, and willing to travel to, correctional facilities
- Minimum of 2 years’ experience working in community corrections or substance use treatment environments;
- Knowledge of working with individuals with co-occurring disorders
- Experience working with and implementing services using Motivational Interviewing and Motivational Counseling Techniques, as well as being familiar with both strengths-based and trauma-informed approaches to treatment.
- Access to vehicle and a valid driver’s license required.
