- Provide client-centered care management, linkage, navigation, and advocacy services to LGBTQ Youth ages 5-29 and their families in Onondaga, Cayuga, Oswego, Herkimer, Madison, and Oneida Counties.
- Maintain an active caseload of 20-25 clients.
- Develop individual care plans in conjunction with care-managed youth and other related service providers, both internal and external. Re-evaluate care plans and conduct case conferencing with other service providers to ensure continuity of client-centered services.

- Provide occasional transportation for clients to appointments.
- Maintain comprehensive, accurate and confidential client files for individualized services provided to youth, including follow-up on testing, treatment, and referrals. Complete necessary paperwork including Ride Logs, and Incident Reports as required. Assist in maintaining complete and accurate statistical information; prepare all required monthly and program reports as assigned.
- Assess education within identified and/or prioritized communities and organizations to increase self-efficacy and independent living skills; reduce social isolation and increase social supports; increase awareness of LGBTQ-affirming providers; promote health and wellness utilizing a risk-reduction/harm-reduction/sex positive approach; and cultivate pride and leadership skills
- Support drop-in and after-school programming for LGBTQ youth at Syracuse and Utica Q Center sites.
- Provide crisis intervention for youth and families and make referrals to internal and external programs/services, as needed.
- Encourage youth participation in program activities and form healthy relationships with youth by being a positive adult role model and mentor, greeting and introducing new members, assisting youth with intake packets, and providing oversight to ensure all youth/young adults are treated with respect and dignity.
- Facilitate/participate in department case conferences as scheduled. Participate in agency Grand Rounds as needed.
- Increase LGBTQ+ cultural responsiveness and competence of area providers and maintain an updated LGBTQ-Affirming Provider list.
- Conduct ongoing assessments with youth, as required by funders, and report findings to LGBT Program Supervisor on a quarterly basis.
- Conduct targeted Outreach to increase awareness and recruit clients for Q Center programs and activities. Attend community provider meetings/committees that address priority population(s) issues and concerns and act as agency liaison to these provider groups when appropriate.
- Maintain complete and accurate statistical information; prepare all required monthly and program reports as assigned; ensure thorough, up to date and complete client records.
- Participate as a culturally competent member of ACR Health, which includes having the capacity to function effectively as an individual within the context of cultural beliefs, behaviors, and needs presented by clients and their communities.
- Incorporate best practices regarding confidentiality into all job duties and communications in accordance with HIPPA, Article 27-F, ACR Health?s policies and procedures and other applicable regulations. Protect agency data in accordance with confidentiality procedures and protocols.
- Assist in the general work of the Youth Services Department, including special projects and general Agency programs.
- Complete/submit all required program and agency documentation within designated time frames.
- Participate in appropriate and applicable training/professional development opportunities for a minimum of 24 hours of curriculum-based training annually.
- Perform other duties as assigned by Director of Youth, Family, & Education Services, Chief Program Officer and/or Executive Director.
