Position Summary:
Our Core Oriented Recovery and Empowerment program is seeking a Psychosocial Rehabilitation Provider. The Psychosocial Rehabilitation Provider helps individuals with overcoming mental health barriers that may have interfered with the person’s ability to function independently and perform normative adult roles in the community. This includes acquiring, re-acquiring, retaining, and improving various life skills such as effective communication and self-advocacy, self-help, self-care, socialization, fine and gross motor skills, mobility, personal adjustment, relationship development, and use of community resources.

Job Responsibilities:

• Provide recovery-oriented activities and interventions that support and restore or build social and interpersonal skills necessary to increase or sustain community tenure, enhance interpersonal skills, establish support networks, increase community awareness, develop coping strategies and effective functioning in the individual’s social environment
• Assist individuals with obtaining personal autonomy: to learn to manage stress, unexpected daily events and disruptions, mental health symptoms, relapse triggers and cravings with confidence; develop and pursue leisure and recreational interests, manage free time comfortably; transportation navigation.
• Assist individuals with effectively advocating for their needs with health care providers; assist with relapse prevention planning, managing chronic medical conditions, mental health symptoms and medications, and establishing good health routines and practices
• Provide wellness coaching such as meal planning, healthy shopping and meal preparation, nutrition awareness, exercise options, appropriate personal care, grooming, sustaining living environment, managing finances and other independent living skills
• Provide onsite modeling, training, and/or supervision to assist individuals in developing maximum independent functioning in community living activities.
• Facilitation of family reunification through coordination of family services as applicable and self-advocacy instruction.
• Provide assistance with increasing social opportunities and developing social support skills that ameliorate life stressors resulting from the individual’s disability and promote health, wellness and recovery.

Qualifications:

• HS diploma or equivalent
• At least one to three years (1-3) of relevant experience working with individuals with substance use disorders and/or Serious Mental Illness or a bachelor’s degree
