Job description
The Project Manager/Grant Writer will work directly with department staff, project stakeholders, and project leadership team to implement grant projects. Grant projects often focus on areas related to community health improvement activities, collective impact, and working across sectors. The Project Manager/Grant may serve internal (Humboldt IPA/NCHIIN) customers or external customers (community partners) to support project work. It’s anticipated that this role will allocate 75% of working time to HCHT project management and 25% of working time to grant writing and grant management. This is a 32 hour a week, grant -funded position. Continuation of this position is contingent on project funding.
Responsibilities:
- Lead planning, design, and implementation activities with the project team
- Help design, coordinate and implement discovery efforts to inform work
- Convene and lead activities and engagement with stakeholder groups
- Design and facilitate meetings pertaining to project work
- Interface with local providers and stakeholders, and serve as the communication lead for the initiative
- Track project progress and ensure that project deliverables are achieved
- Develop communication materials to convey project work, including presentations, documents, etc.
- Attend meetings, conferences, webinars, etc. on behalf of the project and report learnings to project team
- Support coordination and scheduling of events/meetings by securing venue, ordering catering, providing meeting materials, writing meeting notes, assembling meeting packets, etc.
- Research grant opportunities
- Write grant applications
- Coordinate with funders, grant partners, and technical assistance providers
- Provide support with reporting, funder communication, and grant management
- Other duties as assigned
Current Project Assignment and Associated Duties:
This position is currently assigned to the Humboldt Community Health Trust- Accountable Communities for Health project. The Humboldt Community Health Trust is a collective impact initiative that works with cross-sector partners to address transformational change. Previous focus areas for the Humboldt Community Health Trust have included substance use disorder and care navigation services. In the third phase of HCHT work, the initiative will continue to focus on community transformation issues and coordinating partners related to youth behavioral health.
Specific duties associated with this project include:
- Managing project work plan and deliverables
- Attending training and learning opportunities through the California Accountable Community for Health Initiative
- Working with leadership to support the HCHT governance committee and various workgroups including: agenda development, meeting facilitation, scheduling, meeting minutes, etc.
- Coordinate and collaborate with local partners pertaining to shared work
- Attend meetings on behalf of the HCHT to support partner alignment and engagement in HCHT efforts
Qualifications:
- Project management experience; experience working on projects across organizations preferred
- Experience and knowledge working in the local healthcare and/or social care community preferred
- Experience working across organizations, with stakeholder groups, and/or on collective impact initiatives
- Experience researching, writing, and managing grants
- Proficient computer skills, including Microsoft Word, Excel, Access, Power Point, and zoom
