Physical Therapist (PT)- F2F & Virtual
POSITION SUMMARY:
The Physical Therapist provides physical therapy (PT) services to students as prescribed by a physician and indicated in their Individualized Education Program (IEP). This includes conducting evaluations, assessments, analyzing data, writing reports, and determining the strengths and areas of concern or need relative to motor abilities to assist students in accessing their educational program.

RESPONSIBILITIES:
- Develop and monitor IEP goals per students needs
- Provide therapy services to assigned students in a school-based setting, develop and practice by planning activities individualized for the skill level of the students to target goal-specific skills identified in the IEP
- Provide direct PT services through individual PT sessions and a collaborative model of consultation with the case manager, parent, and other related service providers (IEP Team)
- Participate in data review Pre-IEP and IEP meetings by identifying skill deficits, recommending assessment needs, consulting with, and assisting the IEP team, including teacher, clinicians, parents, school personnel, in the development of the PT goals
- Writes evaluation reports if required identifying the students needs and proposed treatment strategies; writes individual student goals and objectives as part of the IEP; maintains progress records.
- Documents treatment plans, IEP Goal, narratives progress notes intervention treatment evaluations, treatment plan reviews, and so forth; and maintains students records and charts in accordance with organizational regulatory accreditations and contractual standards.
- Adhere to Attain ABA and school established timelines relating to all required documentation
- Adhere to laws, policies, procedures, and ethical standards of relevant State Board and any regulatory authorities.
- Perform other duties as assigned.

QUALIFICATIONS AND REQUIREMENTS:
- Graduate of an accredited university with a Masters degree.
- Physical therapist license in state of employment.
- Must have current, valid drivers license in state of residence, and acceptable driving record.
- Must adhere to and complete all requested ongoing CEUs, training, and certification renewals.
