The Physical Therapist evaluates the needs of assigned Home Health patients establishing a plan of care and goals with the patient and provides interventions to assist in achieving independence with functional mobility. Effectively interacts with patients & care givers, co-workers, physicians and other healthcare members.

• Develop and implement a plan of care that meets each patient's specific needs according to supervising MD orders and with agency policies and procedures.

• Responsible for planning, coordinating and documenting care for an assigned caseload of patients.

• Proficient and Experienced in Axxess EMR Software.

• Collaborate with other team personnel to achieve well-rounded care.

• A great bedside manner and a professional attitude

• Responsible and have an extreme ability to work independently

QUALIFICATIONS

• Professional License (PT)
• Valid CPR card
• Valid Driver’s license
• Valid Car Insurance
