Full-time position.Lived (mental health and/or substance use disorder) experience required; Bachelor's Degree in Human Services field preferred; Associates Degree or minimum of two years working with chronic mentally ill and/or substance use disorder population required.

Providing a vast continuum of behavioral health care in a variety of settings, including outpatient, primary care offices, intensive outpatient, inpatient psychiatry, Harford County's crisis center, and the emergency department, you will be able to thrive and grow professionally with our strong leadership team.

The Community-Based Peer/Care Coordinator works in collaboration with the multi-disciplinary team in order to identify and link patients with appropriate community-based resources for aftercare with the goal of continued progress towards recovery. Identifying and addressing barriers to recovery (from mental health and/or substance use disorder symptoms) including, but not limited to, ineffective support system, finances, housing, and lack of transportation to increase likelihood of outpatient treatment engagement.

Maintains ongoing professional relationships with community organizations for ease of referrals, etc. Facilitation of 1:1 meetings with clients, peer support groups, activities that reduce isolation and develop self-advocacy skills and the development of recovery plans to ensure clients are able to maintain independently in the community.
