Position Description
- Provide comprehensive psychosocial and functional assessments of aging and disabled clients.
- Evaluating safety assessments, including inside and outside of the home.
- Assessing fall risks utilizing appropriate tools.
- Participate in consultation with health care individuals, including physicians and nurses.
- Monitoring clients thru monthly contacts and yearly comprehensive assessments.
- Determining the different levels of the Dementia and Alzheimer’s clients in order to access their individual needs.
- Understanding Geriatric medications, especially usable diagnosis for elder patients.
- Developing appropriate care plans for frail, memory deficient clients, and monitoring them on a monthly basis.
- Work collaboratively with Swedish Providence Hospital’s Interns and Doctors on client progression on a monthly basis.
- Advocate with governmental agencies to get help clients get appropriate service’s eligibility.
- Access the Mental Health status tool to determine client’s mental capacity for services.
- Recognizing client’s competency and culture.
- Understanding the developmental phases of aging.
- Understanding the role of religion and spirituality.
- Working cooperatively with research issues and researchers at UW School of Public Health and the UW School of Nursing.
- Participate with AAEP Program nurse on consultation with the frailest clients.
- Access to and protection of PHI/ePHI:
- Responsible for upholding a culture of privacy and security in highly confidential work environment (HIPAA covered entity service area).
- Complies with all CCSWW policies and procedures that involve access to and safeguarding of client Protected Health Information (all forms).

Job Conditions
Working conditions include working in an office setting using the phone and computer and documenting information in client files. Conditions would also include driving to clients homes to do assessments. Regular work hours could include working from 8:00-4:30 Monday through Friday or if part time it could be a variation of those hours.
Physical And Mental Acuity Requirements
- Able to hear telephone rings, phone conversation and face-to-face conversation, door bells and emergency alarms.
- Able to speak clearly in person and on the telephone.
- Able to hand write legibly.
- Vision that enables person to read normal size print and handwritten notes; and distance and peripheral vision than enables person to monitor living spaces and identify concerns that require staff response.
- Able to sit for sustained periods of time.
- Able to walk the entire building and ascend and descend stairs quickly to move from one floor to another in order to respond to emergency situations.
- Mobility/dexterity of hands/arms to enable keying into a unit or other locked area as well as using computers and other office equipment.
- Ability to prioritize multiple tasks, and to work independently and as a team member.
- Ability to lift, move and/or carry up to 40 pounds.
- Regularly able to perform duties as assigned.
- Able to make independent decisions and apply sound judgment in performing job duties.
- Bachelor’s degree in Social Services, Psychology, Counseling, Nursing, Sociology, Public Health or related/transferable fields.
- Three years full time, paid experience providing direct social or mental health services in any human services field including services involving mental health counseling, medication monitering or crisis intervention.
- Experience conducting comprehensive medical/psychosocial assessments while administering and monitoring service delivery.
- Required to complete Family Care Giver support training.
- Experience working with elders experiencing chronic or minor depression.
- Experience developing and implementing individualized service plans with ability to maintain confidential records.
- Demonstrated proficiency and use of electronic mail and industry standard computer software including Microsoft Office (i.e. Word, Outlook, Excel, etc.)
- Ability to work effectively within diverse work groups and populations with proven ability to collaborate with community based and government agencies.
- Comfortable working in a fast-moving collaborative team-oriented environment.
- Support and uphold the mission, beliefs and values of Catholic Community Services and Catholic Housing Services and knowledge of and commitment to working within the Social Teaching of the Catholic Church.

- Applicant must successfully pass required background checks prior to an offer of employment.
- Washington State Driver’s License and automobile insurance.
- Master’s degree in Social Work, Psychology, Counseling, Nursing, Sociology, Public Health or related/transferable fields.
- Experience working in major social service, health and/or government agencies.
- Experience working with African American elders who have unstable health conditions and risk factors including multiple physical care needs.
