Job Summary
The Intensive Outpatient Clinic is a primary care medical home for medically and behaviorally complex patients within the U of U Healthcare system. Our integrated team of medical and behavioral health professionals care for patients in a way that addresses medical and mental health issues, as well as social determinants of health.
Coordinates services as directed by attending or referring physician or clinical case manager. Oversees delivery of individualized services to patients and families.
This position is responsible for the coordination of care activities under the direction of a clinic manager. Interacts with representatives from insurance companies, health maintenance organizations, home care agencies, skilled nursing facilities, durable medical equipment companies, and transportation agencies. Will assist patients with housing, food security, finances, and transportation when needed, along with helping patients address other social determinants of health. This position provides exceptional care for our patients and guests by offering them professional service and creating a memorable patient experience. The incumbent accurately and professionally handles clinic operations tasks including, but not limited to, answering phones, patient registration and scheduling processes.

Essential Functions
- Functions as the patients’ advocate to facilitate effective communication and appropriate treatment.
- Orients patients to the Health Sciences Center program.
- Works with health care team, including the patient in development and implementation of the care plan.
- Advises patients and families regarding financial matters; in accessing financial and community resources.
- Assists in establishing and maintaining good relationships with referring physicians, third party payers and community agencies.
- Facilitates involvement of appropriate treatment team members, timely completion of evaluations and reports and scheduling of team conferences.
- Maintains contact with patients’ family in order to facilitate family involvement and apprise the family of patient progress.
- Implements patient discharge plans. Coordinating with home care agencies, post-acute care facilities, durable medical equipment companies, transportation agencies and others as indicated.
- Collaborates with team to confirm authorization for services are in place.
- Prepares documents and reports required for internal and/or external groups in the scope of care coordination activities and/or data collection.
- Documents activities and progress in medical charts, computer billing/utilization systems, or in other ways as directed.
- Inputs clinical information into computer database.
- Schedules patient appointments and contacts patients for rescheduling, missed appointments and appointment reminders.
- Acts as a liaison between patients, guests, back office staff and providers.
- Performs other duties, as required.

- Demonstrated knowledge of HIPAA regulations to ensure that patient information is guarded and respected.
Minimum Qualifications
Bachelor’s degree in a clinical area, health care administration, or related field, or equivalency (one year of education can be substituted for two years of related work experience); two years of experience in case management activities with third party payers; and demonstrated organizational, human relation and effective communication skills required. Licensure to practice in a clinical field preferred.
