The Patient Care Coordinator is responsible for assuring that all patients are connected to healthcare coverage and services, community resources, referral to appropriate medical offices and any other relevant service as identified as part of the assessment and care planning process.
Additional responsibilities include:
- Identify appropriate resources to ameliorate areas of need, beginning with the most urgent.
- Assure that all members have a primary care provider and remove barriers to receiving preventative care services.
- Track referrals and outcomes
- Based on assessment results, determine, and facilitate care coordination activities and develop a patient centered plan of care
- Obtain necessary consents and releases to enter working relationship and to facilitate referrals and linkages to resources and services.
- Assess eligibility for and assist uninsured members with obtaining health insurance, including obtaining access to health insurance through the New York State of Health Marketplace and several NYS Medicaid programs.
- Take necessary steps to link individuals to community resources, being cognizant of the individual’s level of activation as it relates to the amount of support required to successfully connect them to care.
- Provide education and assistance to uninsured and underinsured individuals
You have:
- High school diploma or GED, plus graduation from a Licensed Practical Nursing Program. Two years’ previous experience as a health care provider.
- Respectful and non-judgmental treatment of all regardless of age, race, class, gender, sexual orientation, religion, ethnicity, national origin or ability.
- Willingness to work a flexible work schedule including evening and weekend hours and possible travel to all centers.
- Ability to establish and maintain effective working relationships with other staff, volunteers, board members, donors, clients and vendors.
What makes you stand out:
- Bi-lingual ability in Spanish or other languages common to the PPGNY service area preferred
Compensation:
- $18-20 an hour

Proof of immunization or immunity to certain communicable diseases (including influenza during the flu season and Covid-19) and testing for tuberculosis. These certifications are required by the NYC DOHMH Health Code, NYSDOH and OSHA.
