High Quality Home Health Care Agency Seeking a Patient Care Coordinator in Auburn, CA

Progressive Home Care is a locally owned home health care agency that has served the Greater Sacramento community passionately for over 20 years. We’re seeking a high energy, organized, and caring Patient Care Coordinator join our team. This is a full-time position. We offer extensive training and a wide array of opportunities for continued education.

Job Description

The Patient Care Coordinator will perform various administrative functions to support our office and clinical team as well as the clinical supervisors. Duties to include but not limited to: nursing scheduling, rehab scheduling, answer questions from clinicians to assist them out in the field, checking frequencies and assisting administrator on call.

Requirements:
- Passionate about high quality patient care!

- Knowledge of the medical field, work experience or classes helpful but will train the right person!
- Good office skills- able to use fax/copy machines
- Organized
- Dependable and motivated
- Excellent communication skills (both verbal and written)
- Self-starter
- High School diploma or higher
- High energy and professionalism a must!

MUST BE AVAILABLE WEEKENDS AND SOME HOLIDAYS!
